After undergoing a cesarean section, itās quite common for women to experience varying degrees of back pain during the postpartum recovery period. For some, the discomfort is mild and short-lived, while for others, it may persist for weeks or even months. This naturally raises an important question ā is back pain after a C-section normal, and if yes, what can be done to relieve it? From hormonal changes and anesthesia to lifestyle adjustments and posture issues, there are several factors that contribute to this condition. In this article, weāll explore the common causes of back pain after cesarean delivery, and offer practical, safe, and doctor-recommended remedies for lasting C-section pain relief.
Back pain after C-section is quite common among women and is usually a result of the bodyās natural response to pregnancy and childbirth. During pregnancy, the body releases hormones that help relax and loosen the joints and ligaments, making it easier to deliver the babyāeven in cesarean births. However, these changes can also make the spine and surrounding muscles more vulnerable to strain. In addition to hormonal shifts, there are several other factors that can contribute to ongoing discomfort in the back after a cesarean delivery.
Here are the most common causes:
Pregnancy hormones, especially relaxin, continue to affect the body even after delivery. These hormonal shifts are a major reason for postpartum back pain, even in C-section cases.
Apart from the visible changes like an increase in the stomach size, there are plenty of invisible changes that the body undergoes during pregnancy. Some of these changes contribute to the causes of back pain after the delivery of the baby.
During pregnancy, the body releases pregnancy hormones and one of them is relaxin. Relaxin helps in the smoother delivery of the baby by loosening the joints and ligaments.
The pregnancy hormones are released regardless of the type of delivery.
After the delivery, as the back joints and ligaments stay in a loose condition, the slightest hint of a heavy task can lead to strain in the back muscles and the patient may face back pain due to it.
The good thing about it is that, with time the back muscles, joints and ligaments gradually gain strength and go back to their normal compact condition.
Weight gain during pregnancy is natural, but the extra load shifts your center of gravity and puts more pressure on the spineāleading to lower back pain after C-section.
Carrying extra weight is another factor that contributes to back pain.
It is normal for a woman to gain weight during pregnancy. After all, the woman grows up to be a whole new person. However, shifting the center of gravity by carrying extra weight towards the front can put stress on the back and spine, which can lead to back pain.
Daily movements like bending, lifting, and holding the baby can cause posture strain. These small adjustments often lead to persistent back pain after cesarean delivery.
The baby might weigh between six to seven pounds. At first, the weight might not seem a lot. But the new activities and adjustments that the mother needs to make like carrying the baby in her arm, bending over to get the baby from the crib or from a car seat, breastfeeding, etc. all these new changes can affect the posture of the woman. Due to the change in the regular posture, the body can put stress on the back muscles which can contribute to back pain. The back pain can sometimes be faced along the spine.
The mother should be more aware of her posture and must do some regular light exercises to keep the back muscles stretched.
Long feeding sessions with improper posture can strain the upper and lower back. Many new moms report C-section back pain worsening during breastfeeding.
During breastfeeding the mother needs to keep herself in a specific posture. Unfortunately, staying in this position for long periods can strain the neck and cause neck pain that spreads to the back. Poor posture during breastfeeding can cause back pain, especially if the baby shrugs.
To relieve pain, the patient can relax their shoulders and place a pillow under their elbows to support their arms.
Spinal or epidural anesthesia used during C-sections can sometimes lead to back pain after surgery, especially if the area is inflamed or irritated.
The type of anesthesia the patient receives during a c-section can be another factor that can lead to back pain after the process. The patient might receive an epidural or a spine block to numb the area.
In epidural anesthesia, the doctor injects the anesthetic around the spinal cord. On the other hand, if the spine is obstructed, the anesthetic is injected closer to the spinal cord. Spinal anesthesia works faster, but epidural anesthesia can take up to 20 minutes to paralyze the abdomen, which can affect the type of administration used.
One of the problems with epidural or spinal cord block is that it can cause muscle spasms near the spinal cord after delivery. These seizures can last for weeks or months after birth.
The patient can perform the following home remedies to get relief from the back pain:
1. Take Warm Showers or Hot Baths: Warm water helps relax stiff back muscles and improves blood circulation, which can naturally reduce inflammation and soreness.
2. Do Gentle Stretching Exercises: Incorporate light postpartum exercises that focus on stretching the lower back, hips, and legs. These movements can improve flexibility and relieve muscle tension. Always consult your doctor before starting any new routine.
3. Maintain Good Posture While Breastfeeding: Poor posture can worsen postpartum back pain. Use a nursing pillow or place a small cushion under your elbow for better arm support while feeding. Keep your spine straight and shoulders relaxed.
4. Get Massage Therapy: A light back massage from a trained professional or a caregiver can help reduce muscle stiffness and tension. Prenatal and postnatal massage therapies are specifically designed to address common postpartum issues like back pain.
5. Take Medications as Prescribed: Over-the-counter pain relievers or prescribed medications can be taken under medical supervision for severe or persistent pain. Avoid self-medication, especially if you’re breastfeeding.
6. Try Physiotherapy for Back Pain Relief: If your pain persists beyond a few weeks, itās best to consult a professional. Physiotherapy for back pain after a C-section focuses on core strengthening, posture correction, and safe recovery exercises that can accelerate healing. A trained physiotherapist can create a customized plan that supports your lower back and overall postpartum recovery.
While home remedies offer short-term relief, managing C-section back pain in the long run requires consistent care and mindful lifestyle choices. Here are some additional tips that can make your postpartum recovery smoother and more comfortable:
Follow a Gentle Recovery Routine
Avoid heavy lifting or sudden movements that strain your core or back. Give your body enough time to heal by listening to pain signals and not rushing your recovery.
Wear a Postpartum Support Belt
A well-fitted maternity or postpartum belt can help support your abdomen and lower back, especially when standing or walking for long periods. It helps redistribute weight and reduces pressure on your spine.
Sleep in a Supportive Position
Sleeping on your side with a pillow between your knees can keep your spine aligned and reduce tension in your lower back. Make sure your mattress offers adequate support.
Include Physiotherapy in Your Recovery Plan
If your pain continues beyond 6ā8 weeks, physiotherapy for back pain after a C-section can be highly effective. A trained physiotherapist can guide you through safe core-strengthening and posture-correcting exercises that promote healing without adding strain.
Stay Hydrated and Eat Nutritious Meals
Healing tissues require proper hydration and nutrients. Focus on foods rich in calcium, magnesium, and anti-inflammatory properties to naturally support muscle and joint recovery.
Take Breaks and Practice Good Posture
New moms often forget to care for their own bodies. Whether you’re nursing, changing diapers, or rocking the baby to sleep ā maintaining good posture is key. Take short breaks, stretch gently, and avoid slouching.
Know When to Seek Medical Help
If your back pain is severe, radiates down your legs, or worsens with time, consult your doctor. It could be a sign of underlying issues that need professional intervention.

Resolve360 is great platform for online physiotherapy.
I was suffering from scapula dyskinesia and had lot of pain in right shoulder movement. I was introduced to Resolve 360 by my oncologist.
Within a month (26 sessions) there was a considerable improvement. Sessions are very cost-effective and doctors are very kind and efficient.
Doctor David has been really great and helpful. I would highly recommend Resolve 360.
I took prenatal care program from Resolve 360. Dr. Preyrna was assigned to me and she has done commendable job. I did prenatal exercises under her supervision, she designed program according to my body need. With her help my pregnancy was healthy and I got good dilations at the time of delivery. I am blessed with a healthy baby girl and very happy with Resolve 360.
Best therapy sessions that too at home and timings are very flexible. I had a issue in my neck and lower back since almost 4 to 5 years, taken so many treatments and physio sessions. But all gave a temporary relief. Then my friend referred me this Resolve 360 in 2020, in starting i was hesitant as everything was online, but after taking almost 10 sessions i got much relief, and now i am almost free of pain. Thanks to the team of Resolve 360.
I had online Physiotherpy sessions by Prerana for my heel pain. ( Tendo Achilis Sprain).
Though virtual, it was more interactive.
She was very observant throughout the sessions, correcting me while doing the exercises and was assertive in her outlook.
I found her instructions and methodology very helpful. She is updated in her academic knowledge in the field of Physiotherpy. She clarified my doubts about the role of Laser and Ultrasound treatment quoting evidence based data. With my personal experience I agree with her opinion that these are more of placebo effect.
Physiotherpy though not a miracle treatment, is a compelling necessity to most of the musculoskeletal and neurological illnesses. It needs to be accepted as healthy way to live.
From being a couch potato with poor cardio respiratory system to reaching pre-athlete levels of fitness in a span of 3 months, my journey with Resolve360 and Dr.David (Physiotherapist) has been anything but magical!
About 3 to 4 months ago, my blood report showed very high bad cholesterol levels, I was low on stamina and had frequent breathing issues. My GP prescribed me cholesterol meds for a month but I felt that alone wouldnāt get me back in shape because I needed a major lifestyle overhaul.
Dr.David educated me about my condition in detail and where it could lead me to if I donāt take action in the long run. He put me on a balanced workout regime being easy with cardio workouts in the initial days and gradually upgraded it observing me closely. He also advises me on dietary choices and follows up frequently. During the initial days, my ears used to get blocked within 5 wall squats and I stop right away but now I do over 20 in one shot and hardly feel a thing. I used to lose balance and fall off over my face within 6 push-ups or 20 seconds of plank and now I do about 24 push-ups in one shot. My heart rate used to raise above 150 bpm doing jumping jacks or other cardio workout within 20-30 seconds. You wonāt believe I donāt do any of these cardio workouts without additional weights on my feet these days and my heart rate and recovery are as steady as they could get. I couldnāt run for over 2 min without stopping and gasping and now I cover about 4.6 km in 30 min and follow-up with a 20 min walk. Not convinced yet? My VO2 Max (I use an Apple Watch to measure this stat) was just about 33 while I started with Resolve360 and David. That reflects a very poor cardio respiratory fitness for a 34 year old. Itās 43.5 today ! Easily above average. Please read upon about VO2 Max and how it directly relates to cardio respiratory fitness so you get an accurate picture about my monumental progress in the last 3 months. Iāve been trying to get into a regular physical workout and balanced diet discipline since my 20s but Iād never been so motivated as Iām now. Give these guys a try. After all, the best investment you could ever make is on your health !
