Sometimes, the sudden causes of back pain may not always be due to any underlying diseases. Back pain can occur due to stress in the muscles caused by overuse of the back during exercising, training, or performing any high-impact task; prolonged sitting, lying, and working in an uncomfortable or wrong posture; prolonged exercising in bad postures; and wearing a poorly fitting backpack regularly.

Back pain is widely seen among individuals over the age of 60 years, with women being more commonly affected. The most common reasons for back pain can be herniated discs, muscle strains, arthritis, back injury, sciatica, spinal stenosis, osteoporosis, fibromyalgia, and many more. In such cases, physiotherapy for back pain is often recommended to relieve discomfort, improve mobility, and prevent further complications.

Primary Causes of Back Pain

1. STRAIN IN THE MUSCLES AND LIGAMENTS: repeated movements to perform a heavy task like lifting loads, repeated bending, etc. can strain the back muscles and the spinal ligaments. Prolonged practice of such tasks can lead to wearing and tearing off the backbone. Due to reduced bone density and bone strength, a simple snap can lead to a back injury.

  • Strain in the muscles and ligaments is often accompanied by muscle spasms and pain along the backbone.
  • Patients with back strain find difficulty in bending, lifting, walking, and working properly.

2. HERNIATED, BULGED, AND RUPTURED DISKS: disks act as the cushions between the units of the backbone called vertebrae. Soft materials are present inside the disks, which can bulge, swell or rupture due to any external factor or disk degeneration with age. The bulged and ruptured disks put stress on the nerves around them, and as a result, the patient may suffer from:

  • Back pain
  • Loss of sensation
  • Swelling of the back
  • Difficulty in maintaining certain postures
  • Difficulty in performing tasks like lifting, bending, sitting, and working.

3. ARTHRITIS: Arthritis is the most prevalent cause of back pain. It is a chronic condition that leads to the inflammation of the joints. An osteoarthritis is a form of arthritis that affects the lower back. The condition can often lead to the narrowing of the space around the spinal cord and lead to a condition called stenosis.

The common symptoms of arthritis are:

  • Stiffness and weakness of the joints and muscles around it.
  • Pain in the affected region.
  • Limited range of movements.

4. OSTEOPOROSIS: osteoporosis is a condition in which the bone becomes weak and fragile. The body constantly absorbs and replaces bone tissue. In osteoporosis, the formation of new bone does not keep pace with the removal of old bone.

Many people are asymptomatic until they experience a bone fracture. Treatment includes medication, a healthy diet, and exercise to prevent bone weakness or to strengthen already weak bones.

5. FIBROMYALGIA: Fibromyalgia is not well understood, but it can cause pain throughout the body, including the back. Researchers are still working to understand the pain of fibromyalgia, but most people agree that it is caused by abnormalities in the nervous system.Back pain has many causes, but it is often treatable and manageable. Lifestyle changes such as learning to lift heavy objects, sleeping carefully in a back-friendly posture, and maintaining a healthy weight can reduce back problems. If the back pain is severe or debilitating, it is better to consult an orthopedist about how to improve the symptoms.

6. STRESS: the spine does an essential job of keeping our body upright. Poor posture and being overweight can put more strain on the spine. When our back muscles have to work extra hard to keep up with the wrong posture, it can lead to back discomfort and pain.

Patients should practice proper working, sleeping, and sitting postures to enhance the back structure.

Secondary Causes of Back Pain

Along with primary causes, secondary causes like age, gender, and family history of the disease also play a major role in defining the extent of the pain and discomfort.

Age: people above the age of 60 years are more prone to develop back pain due to any underlying diseases or sudden muscle strain.

Gender: back pain is more common in women than in men.

Family history: patients with a personal and family history of any disease that leads to the wear and tear of the back, have a higher risk of attaining back pain.

Medication:

Nonsteroidal anti-inflammatory drugs (NSAIDs) and acetaminophen (commonly available as paracetamol in India) are two types of readily available pain relievers that are frequently used for back pain treatment. Both have some side effects which need to be kept in mind. . Before taking any medication, it is important to consult a doctor. Also, it is important to not expect the medication to solve your pain problem on its own. Any backache will most likely require more than one type of treatment to ensure maximum relief

Physical Therapy:

Physical therapists can teach you how to sit, stand, and move in a way that keeps your spine aligned and relieves back strain. Physiotherapy for back pain not only provides immediate pain relief but also helps prevent future back issues. During the sessions, therapists guide you through specialized exercises designed to strengthen the core muscles that support your back. A strong core is one of the best ways to avoid future backache. Back pain can be reduced by improving your strength, flexibility, and endurance through physiotherapy, but it requires consistency and time.

Good Sleep:

Sleeping can be difficult when you have back pain. It can be a vicious cycle because not getting enough sleep can in turn make your back pain worse. The intensity of back pain increases by sleeping in an uncomfortable position. It is advised to start lying on your side while sleeping. Putting a pillow between your knees will keep your spine in a neutral position and also relieve back strain. So if you must sleep on your back, place a pillow between your knees. It is equally important to make sure you sleep on a mattress that is firm enough for your back to remain straight.

Good posture:

We all know that slouching (sitting or standing with a bent back) is harmful to our back. In addition, poor posture can actually increase back pain, especially if you sit for long periods of time. So make sure you don’t hang down over your computer. Maintaining good posture is an important part of back pain treatment. You can start by trying to sit up straight, with your shoulders relaxed, and your back against the back of your chair. Placing a pillow or rolled towel between your lower back and the seat can also help. Maintaining your feet flat on the floor can also help ensure good posture.

Cognitive Behavioral Therapy (CBT):

CBT is a method of psychotherapy. By encouraging new ways of thinking, CBT can help manage chronic back pain. It could include relaxation techniques as well as methods for maintaining a positive attitude. According to studies, patients who receive CBT tend to become more active and exercise, resulting in a lower risk of back pain recurrence.

Diet :

Some diets, mainly those high in trans fats, refined sugars, and processed foods, are extremely inflammatory. Make sure you consult your doctor to determine whether your diet is contributing to your chronic back pain and how you can change it. Maintaining a healthy weight may also help alleviate back pain by relieving the pressure on the spine.

Heat and Cold Application:

A heat pack applied to the affected area can help relieve pain and stiffness. You can use a reusable heat pad, which can be purchased at pharmacies, and or a hot-water bottle. A hot bath or shower can have a similar calming effect. Alternating between heat and ice therapy throughout the day or week may be beneficial. You could try hot and cold packs to see what works best for you. To avoid skin burns or irritation, keep your skin away from direct contact with heat or ice packs. Applying ice or heat for 15 to 20 minutes at a time is usually sufficient.

Pain management programs may provide back pain relief by assisting you in controlling your pain and teaching you how to live with chronic pain. They are usually sessions in which you learn about pain not only from a physical standpoint but also how it affects your mood and emotional well-being.
