Ankylosing spondylitis is an auto-inflammatory disease. The immune system mistakenly causes chronic inflammation in joints and ligaments

3. ENVIRONMENTAL TRIGGERS

Gut bacteria imbalance can play a role in AS (research ongoing). Certain infections can trigger AS in genetically predisposed individuals. 

Ankylosing Spondylitis Symptoms :

The symptoms of Ankylosing Spondylitis vary and often develop slowly.

1. Manual Therapy

Manual therapy applies hands-on treatment by physiotherapists to relieve pain and improve mobility by reducing joint stiffness and muscle tension. 

It consists of:

  • Hands-on technique : The therapist uses only their hands and not machines for working on your muscles, joints and soft tissues.
  • Joint mobilization or manipulation: Joint mobilization and manipulation with the help of controlled and guided movements can help increase range of motion and reduce joint stiffness. 
  • Soft tissue massage : Tissue massage and myofascial release can help relieve tension in the muscles and other surrounding tissues. 
  • Spinal mobilization : Spinal mobilization greatly helps reduce back pain and improves flexibility.

4. Pain-Relief Modalities 

These are non exercise treatments that can relieve pain and support healing. 

These consist of: 

  • Heat/Cold therapies: Applying heat through warm baths or heating pads can increase blood flow in any region and relaxes muscles along with connective tissues.

On the other hand, applying ice packs can decrease blood flow to a region and reduce swelling and inflammation. 

Heat is better for chronic pain and stiffness while ice is good for acute pain and inflammatory cases.

  • TENS(transcutaneous electrical nerve stimulation): A portable device sends low intensity electrical signals so that the brain perceives pain as tingling. This method isn’t directly helpful in AS but manages pain.
  • Ultrasound therapy : Sound signals in ultrasound therapy heat the deep tissues and help reduce pain and relax the muscles.

None of these modalities treat AS directly but help in pain relief only.

5. Aquatic Therapy 

Aquatic therapy also called Hydrotherapy or Water-Based Therapy are exercises performed under water so that the pressure on the spine is reduced to improve strength and flexibility. 

Water provides several effects such as buoyancy, warmth and hydrostatic pressure that aid in easier movements for patients that usually experience restricted range of motion.

Aquatic therapy is usually done alongside other physiotherapy treatment techniques for optimum results. 

6. Education and Lifestyle Strategies 

Understanding about your condition and recognizing how Ankylosing Spondylitis affects your body can help you get a better understanding of your treatment plans. 

Under educational and lifestyle strategies, the patient is guided on:

From being a couch potato with poor cardio respiratory system to reaching pre-athlete levels of fitness in a span of 3 months, my journey with Resolve360 and Dr.David (Physiotherapist) has been anything but magical!

About 3 to 4 months ago, my blood report showed very high bad cholesterol levels, I was low on stamina and had frequent breathing issues. My GP prescribed me cholesterol meds for a month but I felt that alone wouldn’t get me back in shape because I needed a major lifestyle overhaul.

Dr.David educated me about my condition in detail and where it could lead me to if I don’t take action in the long run. He put me on a balanced workout regime being easy with cardio workouts in the initial days and gradually upgraded it observing me closely. He also advises me on dietary choices and follows up frequently. During the initial days, my ears used to get blocked within 5 wall squats and I stop right away but now I do over 20 in one shot and hardly feel a thing. I used to lose balance and fall off over my face within 6 push-ups or 20 seconds of plank and now I do about 24 push-ups in one shot. My heart rate used to raise above 150 bpm doing jumping jacks or other cardio workout within 20-30 seconds. You won’t believe I don’t do any of these cardio workouts without additional weights on my feet these days and my heart rate and recovery are as steady as they could get. I couldn’t run for over 2 min without stopping and gasping and now I cover about 4.6 km in 30 min and follow-up with a 20 min walk. Not convinced yet? My VO2 Max (I use an Apple Watch to measure this stat) was just about 33 while I started with Resolve360 and David. That reflects a very poor cardio respiratory fitness for a 34 year old. It’s 43.5 today ! Easily above average.
