What Is Physical Therapy After Hip Replacement?
Physical therapy after hip replacement is a structured rehabilitation program that begins within 24 hours after surgery and continues for 6 to 12 weeks for most Americans. The program focuses on restoring hip range of motion, rebuilding the strength of surrounding muscles, reducing post-surgical pain and swelling, and safely returning you to daily activities like walking, climbing stairs, getting in and out of your car, and eventually sports or an active lifestyle. Recovery progresses through three main phases — early (Weeks 1–2), intermediate (Weeks 3–6), and advanced (Weeks 7–12). The American Academy of Orthopaedic Surgeons (AAOS) recommends that all total hip replacement patients participate in a guided physical therapy program to maximize their long-term outcomes and reduce the risk of complications. At Resolve360, our online physical therapy program for hip replacement delivers expert, personalized care to US patients in all 50 states — without leaving home.
Every year, more than 450,000 Americans undergo total hip replacement surgery — and that number is growing every year as the population ages. Hip replacement is one of the most successful orthopedic procedures in modern medicine. But here’s what your surgeon doesn’t always have time to fully explain during a short post-op visit: the surgery is only half the battle.
Without a structured hip replacement physical therapy program, your brand-new joint can stiffen up quickly. The muscles surrounding your hip — already weakened by years of arthritis pain and then further disrupted by surgery — cannot simply switch back on by themselves. Left without proper rehabilitation, this leads to a longer and more painful recovery, greater fall risk, chronic hip weakness, and in some cases, joint dislocation.
The American Academy of Orthopaedic Surgeons (AAOS) and the American Physical Therapy Association (APTA) both recommend beginning physical therapy for hip replacement within 24 hours of surgery. This is not optional follow-up care — it is a core, essential part of your surgical recovery process.
Research published in the Journal of Arthroplasty found that hip replacement patients in the United States who completed a structured physical therapy program had a 40% lower rate of post-surgical complications and were discharged from home health care an average of 3 weeks earlier than patients who skipped or delayed their PT program.
This surprises most American patients: physical therapy after hip surgery starts the same day or within 24 hours of your procedure — while you’re still in the hospital. A physical therapist will come to your room to begin basic, controlled movements. Not to push you through a tough workout, but to get the critical healing process started immediately.
These first hospital sessions focus on three urgent priorities:
After hospital discharge — typically 1–3 days for most Americans — your PT continues either at an inpatient rehab facility, an outpatient clinic, or through an online telehealth platform like Resolve360, where our licensed physical therapists connect with you via live video sessions directly from your home.
Physical therapy for hip replacement follows a proven, evidence-based progression. Think of it like building a house — you need a rock-solid foundation before adding walls and a roof. Rushing ahead or skipping phases leads to setbacks that cost you more time in the end.
Goals: Control swelling, prevent blood clots, restore basic safe movement
Goals: Build hip muscle strength, improve range of motion, transition to cane
Goals: Walk independently, improve balance, return to daily life
Goals: Return to sports, work, and full lifestyle activities
The most searched question about hip replacement recovery in the US is: “How long is physical therapy after hip replacement?” Here’s the straight answer: 6 to 12 weeks is the standard range for most Americans — but your individual timeline depends on several key factors.
For most Americans, the answer is yes — physical therapy after hip replacement is covered by insurance. Here’s how coverage typically works across the major US plans:

Real recovery stories from real patients are among the most valuable information you can find. Here are three genuine recovery journeys from Resolve360 patients across the United States:
Robert had been living with severe right hip osteoarthritis for nearly three years. In March 2024, he underwent a total right hip replacement in Dallas and was advised to begin PT within 24 hours.
Living 45 minutes from the nearest clinic, he chose Resolve360’s online program. By Week 4 he transitioned to a cane. By Week 7 the cane was gone. By Week 12 he completed a 2-mile outdoor hike near Lake Texoma.
Sandra was initially skeptical about online PT after surgery. She worried recovery would be slow and not as thorough as in-person care.
Her therapist monitored gait in real time, corrected movement patterns, and educated her thoroughly on hip precautions. By Week 8 she was walking independently.
James, a recreational tennis player, feared he might never return to sport after surgery. His therapist built a sport-specific recovery plan from Day 1.
By Month 3 he resumed cycling and agility drills. By Month 6 he returned to competitive doubles tennis in Arizona.
Telehealth physical therapy has transformed post-surgical recovery for Americans from coast to coast. Here is why more and more US hip replacement patients are choosing online PT over traditional clinic visits:
There are dozens of PT options out there. Here's exactly why thousands of Americans recovering from hip replacement surgery trust Resolve360.

Flexible scheduling across all US time zones — morning, afternoon, or evening sessions.
You work with one dedicated therapist who understands your history and progress.

Every session tracks pain, ROM, strength, and walking quality — clear measurable progress.
Therapists coordinate directly with your orthopedic surgeon when needed.
Over 500 surgeons and orthopedic practices refer post-surgical patients to Resolve360.
After hip replacement surgery, your orthopedic surgeon gives you a set of “hip precautions” — movement restrictions that protect your new joint from dislocating while the soft tissue heals and the implant stabilizes in the bone. Your physical therapist at Resolve360 reinforces and monitors these precautions throughout every session.
Standard precautions for posterior approach hip replacement (the most common technique in the US):
If you had an anterior approach hip replacement, your precautions are often significantly less restrictive — but always confirm the specific list with your surgeon, not a general internet search. Your Resolve360 therapist will know exactly which precautions apply to your specific surgery type.
These are the habits that our most successful US patients consistently follow — the ones who hit every milestone on time or early:
The vast majority of American hip replacement recoveries go smoothly when patients follow their physical therapy program. But knowing the warning signs that require immediate medical attention can be the difference between a minor complication and a serious emergency.
These symptoms may indicate a hip dislocation, joint infection, or dangerous blood clot. Do not wait and see — contact your surgeon or emergency room the same day.

Resolve360 is great platform for online physiotherapy.
I was suffering from scapula dyskinesia and had lot of pain in right shoulder movement. I was introduced to Resolve 360 by my oncologist.
Within a month (26 sessions) there was a considerable improvement. Sessions are very cost-effective and doctors are very kind and efficient.
Doctor David has been really great and helpful. I would highly recommend Resolve 360.
I took prenatal care program from Resolve 360. Dr. Preyrna was assigned to me and she has done commendable job. I did prenatal exercises under her supervision, she designed program according to my body need. With her help my pregnancy was healthy and I got good dilations at the time of delivery. I am blessed with a healthy baby girl and very happy with Resolve 360.
Best therapy sessions that too at home and timings are very flexible. I had a issue in my neck and lower back since almost 4 to 5 years, taken so many treatments and physio sessions. But all gave a temporary relief. Then my friend referred me this Resolve 360 in 2020, in starting i was hesitant as everything was online, but after taking almost 10 sessions i got much relief, and now i am almost free of pain. Thanks to the team of Resolve 360.
I had online Physiotherpy sessions by Prerana for my heel pain. ( Tendo Achilis Sprain).
Though virtual, it was more interactive.
She was very observant throughout the sessions, correcting me while doing the exercises and was assertive in her outlook.
I found her instructions and methodology very helpful. She is updated in her academic knowledge in the field of Physiotherpy. She clarified my doubts about the role of Laser and Ultrasound treatment quoting evidence based data. With my personal experience I agree with her opinion that these are more of placebo effect.
Physiotherpy though not a miracle treatment, is a compelling necessity to most of the musculoskeletal and neurological illnesses. It needs to be accepted as healthy way to live.
From being a couch potato with poor cardio respiratory system to reaching pre-athlete levels of fitness in a span of 3 months, my journey with Resolve360 and Dr.David (Physiotherapist) has been anything but magical!
About 3 to 4 months ago, my blood report showed very high bad cholesterol levels, I was low on stamina and had frequent breathing issues. My GP prescribed me cholesterol meds for a month but I felt that alone wouldn’t get me back in shape because I needed a major lifestyle overhaul.
Dr.David educated me about my condition in detail and where it could lead me to if I don’t take action in the long run. He put me on a balanced workout regime being easy with cardio workouts in the initial days and gradually upgraded it observing me closely. He also advises me on dietary choices and follows up frequently. During the initial days, my ears used to get blocked within 5 wall squats and I stop right away but now I do over 20 in one shot and hardly feel a thing. I used to lose balance and fall off over my face within 6 push-ups or 20 seconds of plank and now I do about 24 push-ups in one shot. My heart rate used to raise above 150 bpm doing jumping jacks or other cardio workout within 20-30 seconds. You won’t believe I don’t do any of these cardio workouts without additional weights on my feet these days and my heart rate and recovery are as steady as they could get. I couldn’t run for over 2 min without stopping and gasping and now I cover about 4.6 km in 30 min and follow-up with a 20 min walk. Not convinced yet? My VO2 Max (I use an Apple Watch to measure this stat) was just about 33 while I started with Resolve360 and David. That reflects a very poor cardio respiratory fitness for a 34 year old. It’s 43.5 today ! Easily above average. Please read upon about VO2 Max and how it directly relates to cardio respiratory fitness so you get an accurate picture about my monumental progress in the last 3 months. I’ve been trying to get into a regular physical workout and balanced diet discipline since my 20s but I’d never been so motivated as I’m now. Give these guys a try. After all, the best investment you could ever make is on your health !
