Online physical therapy can help some people with sciatica. During a live video visit, a licensed physical therapist can review your symptoms, watch how you move, and pinpoint which positions and activities make your leg pain better or worse. From there, they can build a home exercise program and adjust it as you improve. What a video visit cannot do is replace every hands-on test, and some symptoms call for an in-person medical or physical exam before rehab starts.
Sciatica describes pain, numbness, tingling, or weakness that travels from the lower back into the leg along the path of the sciatic nerve. It is a symptom, not a diagnosis, so good care begins with understanding what is irritating the nerve. This guide explains how virtual care works, what the evidence does and does not show, and how to decide whether online physical therapy for sciatica is a sensible place to start.
For many people, yes. Most physical therapy for sciatic nerve pain is built on three things: assessment, education, and progressive exercise. All three can be delivered over video. A physical therapist can observe your movement, find positions that ease or aggravate symptoms, teach a home exercise program, and change it as you progress.
That matters because exercise sits at the center of sciatica rehabilitation. The 2021 clinical practice guideline for low back pain, published in the Journal of Orthopaedic & Sports Physical Therapy (JOSPT) with the Academy of Orthopaedic Physical Therapy, supports exercise training, including trunk strengthening, endurance work, and specific trunk muscle activation, for people with acute low back pain that travels into the leg (George et al., 2021).
There is also direct evidence that physical therapy helps sciatica itself. In a randomized trial of 220 adults with sciatica of less than 90 days’ duration, people referred to four weeks of physical therapy had less disability and lower back pain intensity at one year than people who received usual care (Fritz et al., Annals of Internal Medicine, 2020). Two caveats are worth knowing. The benefit was described as modest, and that program was delivered in person and included manual therapy. A video visit can deliver the exercise and education parts of that care, but not the hands-on techniques.
Virtual physical therapy for sciatica usually follows the same sequence as a clinic visit: an intake and safety screen, a guided movement assessment, a personalized plan, coached exercise, and scheduled follow-ups to track progress. The difference is that your therapist watches and coaches through a camera instead of standing beside you.
Your physical therapist reviews your health history, how and when symptoms started, what you have already tried, and your goals. They also ask screening questions about warning signs that should mean medical attention first.
You are guided through movements such as bending, standing from a chair, walking, and balancing on one leg while the therapist watches and asks how your back and leg respond. A phone or laptop placed so your whole body is visible makes this far more useful.
Based on what they see, the therapist sets goals and chooses a starting set of exercises, along with practical changes to sitting, sleeping, lifting, or work routines.
You practice each exercise live while the therapist corrects form, adjusts range or effort, and checks that symptoms are not spreading further down the leg.
Later sessions track changes in pain, walking and sitting tolerance, and daily function. The plan is progressed, simplified, or, if needed, paused with a referral for in-person evaluation.
A physical therapist can assess a lot over video: where your pain travels, how symptoms respond to movement, posture, mobility, balance, and how sciatica affects daily tasks. Video cannot reproduce hands-on testing such as reflex checks, resisted strength grading, or detailed sensation testing.
When a finding needs hands-on confirmation, a careful online physical therapist will recommend an in-person assessment rather than guess.
Online physical therapy for sciatic nerve pain is largely active care: you do the work between sessions, and your therapist shapes and progresses it. A typical plan draws on several of these elements, chosen for your symptoms rather than applied as a package.
This is where physical therapy for sciatica at home has a practical advantage: your therapist sees the chair you actually sit in, the bed you get out of, and the space you exercise in, which makes advice easier to apply.
The evidence is encouraging but incomplete. Studies of telerehabilitation for low back pain generally show improvements in pain and function similar to conventional rehab, while research focused specifically on virtual physical therapy for sciatica remains limited. Results depend on the cause of symptoms, supervision, and how consistently exercises are done.
Here is what recent, peer-reviewed research shows, and where it stops:
The honest reading: online physical therapy for sciatica looks like a reasonable, often comparable option, not a superior one. Most trials studied chronic low back pain in general rather than confirmed nerve-root symptoms. Live supervision, exercise consistency, symptom severity, and the underlying cause all appear to shape results.
Online physical therapy for sciatica tends to suit people whose symptoms are stable or improving, who can move safely on camera, and who mainly need assessment, exercise guidance, and regular follow-up rather than hands-on treatment or an urgent medical workup.
That often includes people who can perform prescribed movements independently, those already checked by a doctor who want structured rehabilitation, and those who struggle to reach a clinic because of distance, work hours, caregiving, or because sitting in a car aggravates their leg pain.
Online care is not the right first step when symptoms suggest significant nerve compression or another serious cause. These warning signs are uncommon, but they call for prompt medical evaluation, and sometimes emergency care, rather than a video exercise session with a physical therapist.
The first group can signal cauda equina syndrome, a rare condition that requires urgent care.
The second list reflects guidance from MedlinePlus (National Library of Medicine) .
None of this means you should fear sciatica. Most cases are not emergencies, and MedlinePlus notes that sciatica often improves on its own, though it commonly returns. A responsible online provider screens for these signs at the start and refers you on when the picture calls for it.
Neither format is universally better. Online physical therapy offers convenience and strong exercise coaching, while in-person care adds hands-on testing and treatment. The right choice depends on your symptoms, clinical needs, access, and the type of assessment or treatment you require.
| Factor | Online physical therapy | In-person physical therapy |
|---|---|---|
| Consultation | Live video from home | Face-to-face in a clinic |
| Home exercise guidance | Strong; coached in your real environment | Strong; usually taught in the clinic |
| Movement observation | Good with clear camera setup | Full view from any angle |
| Hands-on techniques | Not available | Available when appropriate |
| Convenience | No travel; easier to fit into a workday | Requires travel and clinic hours |
| Equipment-based assessment | Limited to what you have at home | Full clinic equipment |
| Certain physical tests | Reflex, resisted strength, and sensory tests are limited | Can be performed directly |
Many people use both: an in-person evaluation to confirm the picture, then online sessions to carry rehabilitation forward. For a broader look, see how physical therapy helps sciatica and lower back pain.
There is no single best sciatica exercise. The right exercises depend on what is irritating the nerve and how your symptoms respond to movement. A physical therapist usually combines mobility work, gentle strengthening, and movement retraining, then adjusts the plan based on your response.
One safety rule applies across all of them: if an exercise makes pain, numbness, or tingling spread further down your leg, or leaves you weaker, stop and tell your therapist. For more on rehabilitation goals and sample exercises, see Resolve360’s guide to sciatica treatment and exercises, and if a disc is involved, physical therapy for a herniated disc.
There is no fixed timeline. Some people notice changes within a few weeks, while others need longer, structured rehabilitation. Recovery depends on the underlying cause, symptom severity and duration, activity level, general health, and how consistently the plan is followed.
For context, the trial described earlier used a four-week program and measured benefits up to a year later, and MedlinePlus advises contacting a provider if an episode lasts longer than four weeks. A good therapist sets expectations early, reassesses regularly, and tells you plainly when another opinion is needed. Be wary of any program that promises sciatica will disappear by a set date.
Choose a licensed physical therapist who is authorized to practice in the state where you are located, has experience with spine and nerve-related conditions, offers live one-on-one video sessions, personalizes your plan, and has a clear process for referring you to in-person care when needed.
Licensure deserves special attention. In the U.S., a physical therapist treating you by telehealth generally must hold a license, or a Physical Therapy Compact privilege, for the state where you are physically located during the session (PT Compact Commission). You can ask for a therapist’s license number and verify it with your state physical therapy board.

According to its website, care is delivered through live one-on-one video sessions with a physical therapist, starting with an assessment of your history and symptoms and leading to a personalized care plan.
After consultations, Resolve360 provides follow-along exercise videos and an action plan through its app, with tools to monitor your progress between sessions. You do not need a referral to book, and the site states that your therapist will tell you if your condition needs hands-on assessment or a doctor’s visit, which matters for sciatica given the warning signs above.
If your symptoms are stable and you want structured, exercise-based online sciatica treatment from home, a first consultation is a low-commitment way to find out whether virtual care is a good fit for you.

Yes. A licensed physical therapist can treat many people with sciatica through live online physical therapy sessions. The therapist can review symptoms, observe movement, provide individualized exercises, and monitor progress. However, online care cannot provide hands-on testing or manual therapy, so some patients may need in-person evaluation.
Online physical therapy can help some people with sciatica through individualized exercises, movement education, and ongoing supervision. Research on telerehabilitation for low back pain generally shows improvements in pain and function, although sciatica-specific evidence remains limited. Results depend on symptoms, underlying cause, supervision, and treatment consistency.
Yes. Many physical therapy exercises for sciatica can be performed at home with little or no equipment. A physical therapist can select appropriate exercises, teach proper technique, and adjust your program as symptoms change. Online physical therapy also allows therapists to provide guidance based on your home environment.
Exercises that may help sciatica include gentle mobility exercises, nerve mobility exercises, core and hip strengthening, and movement retraining. There is no single exercise that works for everyone. A physical therapist can recommend exercises based on your symptoms and movement response, while avoiding activities that worsen or spread symptoms.
A virtual physical therapy assessment takes place through a live video appointment. The therapist reviews your symptoms, medical history, daily activities, and goals before observing movements such as bending, walking, standing, or balancing. The therapist uses these observations to develop an individualized rehabilitation plan and identify situations requiring in-person evaluation.
Online physical therapy may be appropriate for people with stable sciatica symptoms when proper screening and professional supervision are provided. Your therapist should identify warning signs before treatment begins. New bowel or bladder problems, saddle-area numbness, significant weakness, or rapidly worsening symptoms require prompt medical evaluation rather than online rehabilitation alone.
There is no fixed number of physical therapy sessions required for sciatica. Treatment needs vary according to the underlying cause, symptom severity, duration, functional limitations, and response to rehabilitation. Some people need only a few supervised sessions with home exercises, while others require several weeks of structured care.
You should seek medical evaluation if sciatica causes significant or worsening weakness, severe pain, fever, unexplained weight loss, or symptoms following major trauma. Seek emergency care for new bowel or bladder problems or numbness around the groin, genitals, inner thighs, or buttocks, which may indicate serious nerve compression.
Sometimes, but online physical therapy cannot replace in-person care for every patient. Virtual sessions can provide assessment, movement observation, exercise instruction, education, and rehabilitation guidance. In-person physical therapy may be necessary for hands-on testing, detailed neurological assessment, manual therapy, or equipment-based evaluation. Some patients use both approaches.
Yes. Telehealth physical therapy is available in the United States, although licensing and practice requirements can vary by state. Before booking, confirm that your physical therapist is licensed where you are located during treatment or has applicable Physical Therapy Compact privileges. Also verify telehealth availability, costs, payment options, and insurance coverage.

I came across a Resolve360 ad and decided to give them a try for my knee pain. I contacted them, and their team got in touch with me and explained everything clearly. I had my online physiotherapy session with Dr. Sandeep, and I had a really good experience. He was patient, friendly, and took the time to understand my pain and daily routine before suggesting exercises. He explained each exercise properly and showed me how to do the movements correctly. I could also ask questions during the session, which made things much easier. I really liked the convenience of doing the sessions from home. I didn’t have to travel to a clinic, and the one-on-one sessions felt personal and comfortable. The follow-up sessions were also helpful and kept me motivated to stay consistent with the exercises. A special thanks to Dr. Sandeep for being so supportive and easy to talk to. He made the whole process feel simple and comfortable. Overall, I’m happy with my experience with Resolve360 and would recommend them to anyone looking for convenient online physiotherapy.Posted on Google![]()

I am currently recovering from ACL reconstruction surgery, and choosing the Resolve360 video rehabilitation platform has been one of the best decisions for my recovery journey. When I joined the platform, I was not only recovering from surgery but was also suffering from severe back pain that significantly limited my mobility. Surprisingly, after just two sessions, I experienced noticeable relief in my back pain, which gave me the confidence to continue my rehabilitation. What I appreciate the most is that every exercise is carefully tailored to my specific condition, recovery stage, and individual needs. The structured rehabilitation plan has kept me consistent and motivated. Without this platform, I don't think I would have had such a well-organized approach to my recovery. A special thanks to Dr. Diksha for her exceptional expertise. She has an incredible ability to understand the root cause of pain and provide effective solutions, even through remote consultations. Beyond her clinical skills, she is incredibly empathetic, patient, and motivating. The Only Solution That Found My Root Cause and Delivered Real Recovery For years, I was trapped in a frustrating cycle, suffering from debilitating back pain and a persistent knee injury. I had seen countless specialists—various physiotherapists and chiropractors—but none of them could provide lasting relief or truly get to the heart of my problems. I was ready to give up. Thankfully, Dr. Apoorva and Dr. Asish insisted I join Resolve 360 for a fresh start on my recovery journey, and it was the best decision I could have made. The true breakthrough came from Dr. David. His diagnostic skill is exceptional; during our dedicated one-on-one video call sessions, he was able to accurately find and address the root cause of both my back pain and knee instability. For the first time, I felt like I wasn't just treating symptoms, but actually correcting the underlying issues. The progress has been incredible. My initial back and knee pain are vastly improved, and Resolve 360 continues to help me, focusing now on making my muscles strong to ensure long-term stability and health. The one-on-one format is the key—it ensures I receive personalized attention, correct technique, and real-time adjustments tailored only to me. I am suffering from heel pain for the last few years and the physiotherapy sessions provided by resolve360 have helped me a lot. The best thing about resolve360 is the flexibility they offer. I had sessions both early in the morning and evening depending upon my schedule. All the doctors on their panel are excellent and are always willing to go extra mile and give more than 100%. They do not prescribe any short cuts and focus on resolving the problem from the root cause instead of promising you any magic pill. I will recommend everyone to give it a try if they are facing any issues related to pain or mobility and is in need of physiotherapy.Posted on Google![]()
