Online physiotherapy, also called virtual physiotherapy or telehealth physical therapy, is care from a qualified physiotherapist delivered through video instead of a clinic visit. In some countries the profession is called physical therapy. The two terms describe closely related services, and this guide uses “physiotherapy” throughout.
Many people assume physiotherapy means hands-on treatment. In practice, a large part of it is assessment, education, and guided exercise. The Chartered Society of Physiotherapy notes that remote delivery can be used for triage, assessment, self-management support, e-rehabilitation and monitoring. Those are the parts online care handles well.
You choose a time, usually through an app, website or phone call. You may be asked about your main problem and how long you have had it. If you have scans or reports, you can share them, but many providers do not require them to get started.
A good provider checks early whether remote care fits your situation. If something needs an in-person doctor or examination, they should say so instead of going ahead.
The physiotherapist takes your history, watches how you move and agrees goals with you. Details are covered in the assessment section that follows.
Based on what they find, they suggest exercises, activity changes and self-care advice. It is built around your symptoms, daily routine and available space at home.

The physiotherapist talks through your symptoms, past injuries, medical history and goals. They then observe you on camera doing movements or daily tasks, explain what they think is going on, and start a plan. It often lasts 30 to 60 minutes.
Every assessment begins with questions: where it hurts, what makes it better or worse, how it affects sleep and work, and what treatment you have already tried. This part is the same as in a clinic and gives a lot of information.
The therapist may ask you to point to the sore area, stand up from a chair, bend, reach overhead, or walk toward and away from the camera. For a knee problem, they may watch you do a small squat or climb a step. They look at how you move, where you hesitate and what reproduces your symptoms.
Imagine someone with a stiff shoulder. The physiotherapist might ask them to raise both arms from the front, then from the side, and compare the two. That comparison, plus the person’s description, helps shape the next step. This is an illustration only, not a template for anyone’s care.
A physiotherapist cannot feel a joint, test muscle tension by hand, or perform manual therapy through a screen. Online assessment gathers what it can and stays honest about what it cannot. Many providers also use the phrase “working impression” rather than a firm diagnosis, since some conditions need in-person testing to confirm.
Most online plans centre on exercise therapy. The physiotherapist shows the movement, watches your attempt, and gives cues such as “slow down” or “keep the knee over the toes”. Some providers then send short videos or instructions so you can repeat the exercises between sessions.
Most exercises need little or no equipment. A chair, a towel or a resistance band may be suggested when useful.

Online physiotherapy does not replace emergency care, a physical examination, imaging or hands-on treatment. NHS England’s guidance on remote musculoskeletal services recommends a blended approach and says some higher-level rehabilitation may be better face to face. It also notes remote assessments can take longer.
Seek urgent medical care, not an online booking, if you have: chest pain or breathlessness; new weakness in a limb; numbness around the groin or inner thighs; new loss of bladder or bowel control; back pain with fever or after a serious fall; a suspected fracture or dislocation; a hot, swollen calf; or sudden severe pain. This list is not complete. If you are worried, contact a doctor or emergency services.
In-person assessment may also make sense when symptoms are unclear, do not improve as expected, need hands-on testing, or when your therapist recommends it. Being referred on is a sign the process is working.
For many musculoskeletal conditions, research suggests supervised online exercise can produce results similar to in-person care. Evidence quality varies by condition, and not every patient or problem is covered. Your assessment determines whether it is appropriate for you.
A 2026 systematic review of upper-limb conditions found similar improvements to in-person rehabilitation, though the certainty of the evidence ranged from very low to moderate. The authors describe it as tending to support telerehabilitation, but not conclusively.
A meta-analysis on tendinopathy (10 studies, 412 people) found a potential advantage over unsupervised home exercise. Another systematic review found comparable satisfaction and similar or better attendance and adherence than in-person physiotherapy.
These findings cannot be stretched to every condition or person. Studies differ in age group, condition, and how much therapist contact was included.
