Online physical therapy is physical therapy delivered through a live, secure video call. A licensed physical therapist (PT) asks about your symptoms, watches you move on camera, and builds an exercise plan you follow at home. Follow-up video visits check your technique, track your progress, and adjust the plan. It can help with many muscle and joint problems, such as back, neck, knee, and shoulder pain. It can’t provide hands-on treatment, and some people need an in-person exam first.

Online physical therapy, also called virtual physical therapy or telehealth physical therapy, is care from a physical therapist delivered through video instead of a clinic visit. The PT evaluates how you move, explains what is likely contributing to your symptoms, and coaches you through exercises in real time.
There are two main formats:
Many providers combine the two, using live visits for assessment and progress checks and an app for exercises between visits.

The first online physical therapy consultation is usually the longest visit. It starts with questions: where it hurts, what makes it better or worse, how it affects your sleep and work, and what you have already tried.

Video has limits: a PT can’t feel a joint, test muscle strength by hand, or perform manual therapy through a screen. If those tests matter for your condition, your PT should tell you and suggest an in-person visit.
For some musculoskeletal conditions, research suggests that therapist-led telerehabilitation can work about as well as in-person care. The evidence is promising but not conclusive:

In-person or hybrid care may also be a better choice if your symptoms are unclear, you aren’t improving as expected, you are at high risk of falling, you need early post-surgery wound or brace checks, or your PT recommends it. Being referred for in-person care is a normal part of good online care.

According to the American Physical Therapy Association (APTA), all 50 states allow some form of direct access to physical therapists, but many limit the number of visits or days before a referral is required. Your insurance plan may also have its own referral rules.
