Who this guide is for: Patients experiencing lower back pain or sciatica who want to safely heal an L4-L5 disc bulge at home without surgery.
The Quick Answer:
Yes, an L4-L5 disc bulge can heal without surgery. The most effective treatment includes expert-guided physical therapy (physiotherapy)—such as the 1-on-1 virtual sessions at Resolve360—along with short daily walks and safe exercises.
Note: Severe warning signs (like loss of bladder control or worsening leg weakness) require immediate emergency medical care, not exercises.

An L4-L5 disc bulge means the disc between your fourth and fifth lumbar vertebrae has spread past its normal edge. It can irritate the L5 nerve root, but many bulges cause no pain at all, so your MRI shows only part of the picture.
An L4-L5 disc bulge is when the disc between the fourth and fifth lumbar vertebrae, in the lower back, spreads beyond its normal edge. The disc’s tough outer ring stays intact. That is the difference from a herniation, where inner disc material pushes through a tear in the ring.
L4-L5 and L5-S1 are the two lowest discs in the spine. They carry the most load and move the most, so they are the most commonly affected levels.
What your MRI does and doesn’t tell you. Disc bulges are very common in people without back pain. A review of 33 studies found bulges on imaging in about 30% of pain-free 20-year-olds, rising to 84% of pain-free 80-year-olds (Brinjikji et al., 2015). A bulge on your report is a finding, not a diagnosis. What matters is whether your symptoms and examination match it.
An L4-L5 bulge usually sends pain down the outer leg to the top of the foot and big toe (L5 nerve). An L5-S1 bulge usually sends it to the back of the calf, heel and outer foot (S1 nerve). Pain that stays in the back is more likely back pain than sciatica.
An L4-L5 bulge can cause back pain alone, or it can irritate a nerve root and cause leg symptoms (sciatica). Common symptoms include:
When leg symptoms appear, where you feel them usually points to the nerve root involved:
| Disc Level | Nerve Root | Where Symptoms Are Felt | Possible Weakness |
|---|---|---|---|
| L3–L4 | L4 | Front of the thigh and inner shin | Difficulty straightening the knee |
| L4–L5 | L5 | Outer thigh and calf, top of the foot, and big toe | Difficulty lifting the foot or big toe, or walking on the heels |
| L5–S1 | S1 | Back of the thigh and calf, heel, and outer foot | Difficulty rising onto the toes |
These patterns overlap, and a clinical examination is needed to confirm them. If your leg pain is the main problem, see our guide to one-to-one online physical therapy for sciatica and leg pain.
Bulges at two or three levels are common on MRI, especially after 40, and they are treated the same way as a single bulge: by your symptoms, not by the number of levels on the report.
MRI reports often list L3-L4, L4-L5 and L5-S1 together because age-related disc changes rarely affect just one level. Most of these findings are also seen in people without pain. What matters is which nerve, if any, your symptoms point to. That is usually one level, even when the scan shows several.
Bladder or bowel changes, numbness in the saddle area or worsening leg weakness need emergency care the same day.
Go to emergency care now (call 911 in the US, or 112 or 108 in India) if you have any of these:
The first two can be signs of cauda equina syndrome, a medical emergency. Exercises and online care are not appropriate until you have been assessed in person.
See a doctor within days if pain is getting steadily worse, numbness is spreading, or you cannot sleep or walk because of the pain.
Yes, in most cases. In a large trial of people with disc-related sciatica, about 95% reported recovery at one year whether or not they had surgery. Surgery mainly made leg pain settle faster.
Low back pain guidelines advise that most acute and subacute back pain gets better over time, and recommend non-drug treatment and exercise first (American College of Physicians, 2017; NICE NG59).
For disc-related leg pain, a large trial compared early surgery with prolonged non-surgical care. Surgery relieved leg pain faster, but disability over one year was similar, and about 95% of people in both groups reported recovery at one year (Peul et al., 2007).
What “healing” means is important. A review of repeat scans found that bulges shrink on imaging much less often than larger herniations: about 13% for bulges compared with 70% for extrusions (Chiu et al., 2015). Pain usually settles anyway, because the irritated nerve and tissues calm down and your back gets stronger. Don’t judge your progress by your next MRI. Judge it by what you can do.
Exercise-based physical therapy and staying active are the first-line treatments. Medicines and injections can ease pain for a while. Traction, back belts and TENS are not recommended.
| Option | What it does | Evidence | Best used |
|---|---|---|---|
| Staying active and walking | Keeps tissues moving and may reduce stiffness. | Encouraging normal activity is consistent with guideline-based low back pain care. | From day one, within comfort. |
| Exercise-based physical therapy | Identifies movements that may ease symptoms and builds strength, control and activity tolerance. | Exercise is recommended in major guidelines, including ACP and NICE, with the approach tailored to the person. | The core of treatment for many people, guided by symptoms and function. |
| Manual therapy (mobilisation, manipulation, massage) | May provide short-term pain relief and make movement easier. | Some techniques may offer short-term benefit. NICE recommends manual therapy only as part of a package that includes exercise. | Alongside exercise, not as a standalone treatment. |
| Heat | May ease muscle tightness and discomfort. | ACP includes superficial heat among options for acute or subacute low back pain. | In the early phase, or before gentle movement, if comfortable. |
| Anti-inflammatory medicines (NSAIDs) | Can reduce pain and inflammation. | Benefits and risks vary. NICE advises considering potential risks and using the lowest effective dose for the shortest possible period. | Short term, after checking with a healthcare professional. |
| Epidural steroid injection | May reduce inflammation around an irritated nerve root. | Evidence suggests a small, mainly short-term benefit for some people with sciatica. See Pinto et al., 2012. | Selected cases of severe sciatica or leg pain; not routine treatment for nonspecific low back pain. |
| Traction, back belts, TENS and ultrasound | Passive treatments intended to relieve symptoms or provide support. | NICE advises against these as routine treatments for low back pain or sciatica. | Generally not recommended as routine treatments. |
Some commonly prescribed tablets for nerve pain (such as gabapentin or pregabalin) and steroid tablets have not shown overall benefit for sciatica, and NICE advises against them. Always discuss medicines with your doctor; this article gives no doses.
Physical therapy finds which movements calm your symptoms, then rebuilds strength and tolerance step by step, which is the approach guidelines recommend first.
A good physical therapy plan is built around your symptoms, not around a scan. Your physiotherapist will usually:
If you want this done with a physiotherapist over video, see physical therapy for disc bulge in the USA or physiotherapy for disc bulge in India.
Start with gentle exercises that keep pain the same or pull it back towards your spine. Stop any movement that sends pain further down your leg.
No exercise suits everyone with a disc bulge. Use this rule for all of them:
Start with 1–2 exercises, a few times a day, and add more as they feel easy.
1. Pelvic tilt. Lie on your back with knees bent. Gently tighten your stomach and flatten your lower back into the floor. Hold 3–5 seconds, relax, and repeat 8–10 times. Useful early on when most movements hurt.
2. Prone press-up (McKenzie extension). Lie face down. Place your hands under your shoulders and gently press your upper body up, keeping your hips on the floor. Lower slowly; repeat 8–10 times. Helpful for many people with disc-related leg pain, but only continue if symptoms ease or centralise.
3. Glute bridge. Lie on your back with knees bent and feet flat. Press through your heels and lift your hips until your body is in a straight line from shoulders to knees. Lower with control; 8–12 repetitions.
4. Bird-dog. On hands and knees, extend one arm forward and the opposite leg back, keeping your back level. Hold 3–5 seconds, then switch sides; 6–8 per side.
5. Side-lying clamshell. Lie on your side, knees bent and feet together. Lift the top knee without rolling your hips back. Lower slowly; 10–15 per side.
6. Walking. Short, frequent walks are often better tolerated than one long walk. Add a few minutes every couple of days if symptoms allow.
Avoid these until your leg symptoms have settled and your physiotherapist clears them. Repeated bending and loaded twisting are the movements most likely to aggravate disc-related leg pain.
For a longer list of options, see our best exercises for lower back pain and disc bulges.
Break up sitting every 30–45 minutes, walk daily, sleep with a pillow between or under your knees, and avoid bed rest beyond a day or two.
Most people feel clearly better within 6–12 weeks. Nerve symptoms such as numbness can take longer, and a full return to heavy work or sport can take 3 months or more.
Everyone’s recovery is different, and flare-ups along the way are common. A typical pattern looks like this:
| Phase | Typical timing | Focus |
|---|---|---|
| Settling | Weeks 0–2 | Calm the pain: find comfortable positions, take short walks and avoid prolonged sitting. |
| Restoring movement | Weeks 2–6 | Try appropriate directional exercises, begin core and hip exercises, and gradually increase walking duration. |
| Strengthening | Weeks 6–12 | Build strength progressively, practise lifting mechanics and return towards normal daily activities. |
| Returning fully | 3 months+ | Gradually return to sport or heavy work and establish a long-term maintenance programme. |
Many people feel clearly better within 6–12 weeks. Nerve symptoms such as numbness can take longer to settle than pain. If you are not improving after about 6 weeks of good care, ask your clinician to reassess you.
Ashish, 38, software engineer. Mark sat at a desk about 9 hours a day and developed sharp sciatica and lower back stiffness. His MRI showed an L4-L5 disc bulge affecting the left L5 nerve root.
| Weeks | What we did | What changed |
|---|---|---|
| 1–2 | Symptom-easing positions, short walks and workstation changes. | Pain became more manageable, although sitting remained limited. |
| 3–6 | Directional exercises, core and glute strengthening, and longer walks. | Leg pain eased and sitting tolerance improved. |
| 7–10 | Progressive strengthening, posture guidance and lifting practice. | Returned to weekend hiking; leg numbness resolved by week 10. |
Surgery is usually considered only when severe leg pain does not improve with non-surgical care, if leg weakness is getting worse, or in medical emergencies like cauda equina syndrome.
The most common operation is a microdiscectomy, which removes the part of the disc pressing on the nerve. While it can relieve leg pain faster, it does not replace the need for rehabilitation. If you do have an operation, check out our guide on physical therapy after back surgery.
Note: Disc bulges and herniations are managed differently in some cases. If your MRI report says “herniation,” “extrusion,” or “protrusion,” read our specific guide: Can physical therapy fix a herniated disc without surgery?

Most people feel significant relief within 6 to 12 weeks with an active, guided physical therapy program. Some cases may take a few months, especially if nerve symptoms (like sciatica) are present. Note: The bulge may still appear on a later MRI even after your pain has completely resolved.
Yes, walking is highly recommended. It is one of the safest and best-tolerated activities for a disc bulge. Start with short, frequent walks on flat surfaces and build up gradually. If you feel pain spreading further down your leg, slow down and consult your physiotherapist.
Most people return to normal activities and stay pain-free. However, back pain can recur. Staying active and keeping up with core strengthening exercises reduces the chances of a flare-up and helps you maintain long-term spinal health.
In a disc bulge, the disc flattens and spreads out, but its tough outer ring stays intact. In a herniated disc (or slipped disc), the inner gel-like material pushes through a tear in the outer ring. Both can cause nerve pain and are usually treated without surgery first.
Studies show that heat therapy is generally more effective for soothing acute lower back pain and relaxing tight muscles. Use a heating pad for 15 to 20 minutes, placing a cloth between the pack and your skin to prevent burns.
Early in your recovery, you should avoid repeated forward bending, sit-ups, and heavy lifting, as these put direct pressure on the L4-L5 disc. You must also stop any exercise or movement that sends pain further down your leg.
Not always. Clinical guidelines advise getting an MRI only if your nerve symptoms are getting rapidly worse, if you have emergency “red flag” signs, or if you are planning for surgery. Many people have disc bulges on an MRI but experience absolutely no pain.
Absolutely. For most people without emergency warning signs, virtual care is highly effective. Platforms like Resolve360 offer live, 1-on-1 video sessions with senior physiotherapists and use patented AI movement-tracking to ensure your exercise form is perfect—helping you heal safely from home.
The best sleeping position is usually on your side in a fetal position with a pillow between your knees. This keeps your spine in a neutral alignment and relieves pressure on the L4 and L5 nerve roots. If you prefer sleeping on your back, place a pillow under your knees. You should generally avoid sleeping on your stomach, as it arches the lower back and increases disc pressure.
Yes, but you must take frequent breaks. Prolonged sitting puts significantly more pressure on your lower lumbar discs compared to standing or walking. If you have a desk job, use a chair with strong lumbar support, keep your feet flat on the floor, and make sure to stand up, walk, or stretch every 30 to 45 minutes.
No, a massage cannot push a bulging disc back into place. While deep tissue massage can provide temporary relief from muscle spasms and stiffness in your lower back, it does not heal the underlying disc issue. For genuine, long-term recovery, you need an active physical therapy program to build core strength and stabilize the spine.

I came across a Resolve360 ad and decided to give them a try for my knee pain. I contacted them, and their team got in touch with me and explained everything clearly. I had my online physiotherapy session with Dr. Sandeep, and I had a really good experience. He was patient, friendly, and took the time to understand my pain and daily routine before suggesting exercises. He explained each exercise properly and showed me how to do the movements correctly. I could also ask questions during the session, which made things much easier. I really liked the convenience of doing the sessions from home. I didn’t have to travel to a clinic, and the one-on-one sessions felt personal and comfortable. The follow-up sessions were also helpful and kept me motivated to stay consistent with the exercises. A special thanks to Dr. Sandeep for being so supportive and easy to talk to. He made the whole process feel simple and comfortable. Overall, I’m happy with my experience with Resolve360 and would recommend them to anyone looking for convenient online physiotherapy.Posted on Google![]()

The Only Solution That Found My Root Cause and Delivered Real Recovery For years, I was trapped in a frustrating cycle, suffering from debilitating back pain and a persistent knee injury. I had seen countless specialists—various physiotherapists and chiropractors—but none of them could provide lasting relief or truly get to the heart of my problems. I was ready to give up. Thankfully, Dr. Apoorva and Dr. Asish insisted I join Resolve 360 for a fresh start on my recovery journey, and it was the best decision I could have made. The true breakthrough came from Dr. David. His diagnostic skill is exceptional; during our dedicated one-on-one video call sessions, he was able to accurately find and address the root cause of both my back pain and knee instability. For the first time, I felt like I wasn't just treating symptoms, but actually correcting the underlying issues. The progress has been incredible. My initial back and knee pain are vastly improved, and Resolve 360 continues to help me, focusing now on making my muscles strong to ensure long-term stability and health. The one-on-one format is the key—it ensures I receive personalized attention, correct technique, and real-time adjustments tailored only to me. I am suffering from heel pain for the last few years and the physiotherapy sessions provided by resolve360 have helped me a lot. The best thing about resolve360 is the flexibility they offer. I had sessions both early in the morning and evening depending upon my schedule. All the doctors on their panel are excellent and are always willing to go extra mile and give more than 100%. They do not prescribe any short cuts and focus on resolving the problem from the root cause instead of promising you any magic pill. I will recommend everyone to give it a try if they are facing any issues related to pain or mobility and is in need of physiotherapy.Posted on Google![]()
