Disclosure: This guide is published by Resolve360, a leading global provider of online physical therapy. We feature our own direct-access service first, and we have a commercial interest in this topic. We evaluated every provider on the same criteria (live 1:1 video, pricing transparency, access and care model) and link to each provider’s official website so you can check the details and make an informed decision.
Who this is for: US patients seeking fast virtual physical therapy.
The Main Answer: The best online physical therapy service depends on your insurance status and how quickly you want to start care.
Whichever you choose, confirm that your therapist is licensed to treat patients in your state.
No Employer or Referral Needed • Free 30–45 Min Initial Assessment
We are your doctor’s first choice and trusted by 50,000+ patients.
Speak to a licensed physical therapist before you commit to anything.
Online physical therapy, also called virtual or telehealth physical therapy, isn’t one kind of service. Some online physical therapy providers connect you with a therapist over live video, some offer app-based exercise programs with a clinician checking in, and a few send a therapist to your home. Below we compare 10 options US patients can use, explain how we compared them, and cover cost, Medicare and state licensing.
Not sure which platform fits your specific needs? Use the comparison table below to quickly evaluate 10 leading virtual physical therapy providers available to US patients in 2026.
We have broken down each service by its core care model, the availability of live 1-on-1 video sessions, published self-pay pricing, and insurance requirements. This side-by-side view will help you easily compare direct-access options with transparent pricing (like Resolve360) against programs that require employer sponsorship (like Hinge Health and Sword Health).

Live telehealth physical therapy. You meet a licensed therapist on a secure video call. They take your history, watch you move, guide tests and exercises in real time, and adjust your plan from visit to visit. This is the closest match to a clinic visit, minus hands-on treatment.
App-guided (digital) programs. These are sometimes called digital physical therapy or digital MSK programs. You follow exercise sessions in an app, often with motion tracking, while a physical therapist or care team reviews your progress and messages you. They’re usually offered as a benefit through an employer or health plan.
In-home and hybrid care. A therapist treats you in person at home and uses an app between visits. Or a clinic lets you mix office visits with video visits. You get hands-on care when you need it, but only where the provider operates.

We included services that US patients can use for musculoskeletal pain or rehab in 2026, as confirmed on each provider’s official website. We left out software built for clinics, any service we couldn’t confirm is currently available to patients, and brands that now belong to another provider on this list. Kaia Health, for example, is now part of Sword. We compared each service on five points:
Resolve360 is featured first because we publish this guide. The other providers are grouped by how you access them and listed alphabetically within each group, because the best choice for someone with an employer benefit is different from the best choice for someone in Utah or someone paying out of pocket. Which one suits you depends on your condition, your eligibility, your location and whether clinicians are available. If you’re an employer comparing programs for your workforce, see our guide to telehealth physical therapy companies for employers.
If your employer or health plan doesn’t cover virtual musculoskeletal care, or you simply want to avoid the delays and high deductibles of traditional insurance approvals, these direct-access platforms let you start therapy immediately. Below are the top self-pay and direct-booking virtual physical therapy options available across the US in 2026.
Resolve360 offers a direct-access virtual physical therapy model that combines live 1-on-1 video sessions with an AI-powered home exercise app. Instead of an aggregator model, Resolve360 works with its own monitored global panel of 108+ qualified physical therapists, including clinicians from the US, UK and India, with 10 to 30+ years of clinical experience.
Learn more about live 1:1 virtual physical therapy with Resolve360.
Agile Virtual PT is a telehealth-only physical therapy practice. Patients get face-to-face video guidance from a licensed physical therapist, and Agile checks your insurance before your first visit.
ATI is a large US outpatient physical therapy chain that also offers online PT. Sessions run over secure video through its CONNECT platform, in a browser or app, with a licensed ATI clinician.
Athletico, an outpatient physical therapy provider, offers virtual visits. A licensed clinician assesses and treats you over secure video, using Microsoft Teams in your browser.
Intermountain Health, a nonprofit health system, offers scheduled 40-minute video visits with a licensed physical therapist for patients in Colorado and Utah.
Luna isn’t a video service. Its licensed physical therapists come to your home or workplace, and you follow your exercises in the Luna app between visits. We include it because many people searching for online PT really want care without driving to a clinic.
These programs are often free or low cost for eligible members, but you can only join if a sponsor covers you. Check your benefits portal or the provider’s eligibility page first.
Hinge Health runs a digital program for joint and muscle pain, pelvic floor symptoms, and balance and mobility. It includes a personalized care plan, exercise sessions tracked by TrueMotion in the app, and support from physical therapists, orthopedic doctors and health coaches.
Sword delivers AI-guided exercise sessions in its app, with a human clinical specialist overseeing your care plan. Sword says the program pairs AI with licensed clinicians. In January 2026 Sword acquired Kaia Health, and Kaia’s US musculoskeletal members are moving to Sword’s platform.
Omada is known for chronic-condition programs, and it also runs a musculoskeletal program delivered with physical therapists through its partner, Physera Physical Therapy Group.
Vori Health offers physician-led virtual care for back, neck, joint and pelvic health. Your care team can include physicians, physical therapists, exercise physiologists, dietitians and health coaches, with live virtual visits and app-guided exercise.
Published self-pay prices among these 10 services range from a free ($0) initial assessment with follow-up sessions starting at $48 (Resolve360), to $100 for an initial visit and $70 per follow-up (Agile Virtual PT), to $135 for an in-home visit (Luna). Many providers don’t publish prices, so ask for the full cost of the assessment, follow-ups, packages and any subscription. Our guide to physical therapy costs without insurance goes into more detail.
Medicare currently lets physical therapists provide telehealth, including to patients at home, through December 31, 2027, under an extension Congress passed in February 2026 (reported by the American Physical Therapy Association). This isn’t permanent. Unless Congress acts again, the Centers for Medicare & Medicaid Services (CMS) says PTs won’t be able to bill Medicare for telehealth from January 1, 2028. Under Part B you generally pay the annual deductible ($283 in 2026), then 20% of the Medicare-approved amount for outpatient therapy. The provider must be enrolled in Medicare and must bill it.
Coverage of telehealth physical therapy depends on your plan, whether the provider is in network, and any visit limits. Call the number on your insurance card and ask: “Is telehealth physical therapy covered, is this provider in network, and what is my co-pay?” Also check your benefits portal for a free digital MSK program.
The IRS allows therapy received as medical treatment as a medical expense (IRS Publication 502), so physical therapy for a medical condition generally qualifies. The rules still depend on your account and plan administrator. Keep itemized receipts and confirm with your administrator before you pay.
In the US, a physical therapist generally has to be licensed, or hold a compact privilege, in the state where you are located during the session, not the state where the therapist sits. The Physical Therapy Compact lets a PT licensed in one member state get a privilege to practice in other member states, but not every state participates. That’s why availability differs: Intermountain serves Colorado and Utah, and Agile says it operates in 41 states. Before your first session, ask:
You usually don’t need a referral to start. All 50 states, Washington, D.C. and the U.S. Virgin Islands allow some form of direct access to physical therapists, though some states limit treatment without a referral, according to the American Physical Therapy Association (APTA). Insurers and Medicare may have their own requirements.
Prefer a local therapist who also offers video visits? APTA’s free Find a PT directory lets you filter for telehealth and home visits.
Research suggests telerehabilitation can be comparable to in-person physical therapy for several conditions, including knee and hip osteoarthritis, low back pain, and rehab after hip or knee replacement. Study quality varies, though, and more high-quality trials are needed (Seron et al., Physical Therapy, 2021). Virtual care works best for exercise-based treatment, education and progress checks. It can’t provide hands-on techniques such as manual therapy, and some exams have to be done in person.
It may suit you if you have common back pain, neck pain, knee pain, shoulder problems such as frozen shoulder, or tennis elbow. It can also help if you’re recovering after hip replacement or other surgery with your surgeon’s clearance, or if clinic visits are hard to fit around work, family or travel. If sciatica is your main concern, read about whether online physical therapy can help with sciatica.
For most people, the best online physical therapy option is the one you’re eligible for that gives you enough clinician contact. If you have an employer or plan benefit, start with Hinge Health, Sword, Omada or Vori Health. If you live in Colorado or Utah, Intermountain is an option, and if you need hands-on care at home, look at Luna where it operates. If you’re paying yourself and want live 1:1 video with a free first assessment, start with Resolve360, and compare it with Agile Virtual PT, ATI and Athletico on price, state availability and licensing.
There isn’t one best service for everyone. If your employer or health plan offers Hinge Health, Sword, Omada or Vori Health, start there, because it may be free. If not, Resolve360 offers direct-access live 1:1 video with a free first assessment. Compare it with Agile Virtual PT, ATI and Athletico on price and state licensing.
For several conditions, including knee and hip osteoarthritis, low back pain and rehab after joint replacement, research suggests telerehabilitation can be comparable to in-person care. Study quality varies, though. Virtual care can’t replace hands-on techniques, so your therapist should tell you if you need an in-person visit.
Prices vary by provider. Agile Virtual PT lists $100 for the first visit and $70 per follow-up, and Luna charges $135 per in-home visit. Resolve360 offers a free ($0) first assessment, with customized therapy plans from $48 per session depending on your condition. Always ask for the full plan cost in writing.
Yes, for now. Medicare lets physical therapists provide telehealth, including at home, through December 31, 2027, according to the CMS telehealth FAQ. You generally pay the Part B deductible ($283 in 2026), then 20% coinsurance. The provider must be enrolled in Medicare. Unless Congress extends it again, this coverage ends January 1, 2028.
Often, but it depends on your plan, whether the provider is in network, and any visit limits. Call the number on your insurance card and ask whether telehealth physical therapy is covered, whether the provider is in network, and what your co-pay will be. Also check your benefits portal for a free digital program.
Usually. Physical therapy for a medical condition is generally a qualified medical expense, but rules depend on your account and plan administrator. Ask for itemized receipts that show the service, and confirm with your HSA or FSA administrator before paying for a package.
Usually not to start. All 50 states, Washington, D.C. and the U.S. Virgin Islands allow some direct access to physical therapists, though some states set limits on treatment without a referral. Your insurer or Medicare may still need a referral or a plan of care signed by a physician. Intermountain’s virtual PT, for example, needs no referral.
Hinge Health says you can’t sign up without a participating employer or health plan. Sword is mainly offered through employers and plans, though its site has an enrollment path for individuals. If neither is available to you, look at services you can book directly and pay for yourself.
Live virtual PT means real-time video sessions with a therapist who watches you move and adjusts your plan as you go. App-based programs guide you through exercise sessions, often with motion tracking, while a clinician reviews your progress and messages you. Live care gives more feedback, while app programs are more flexible.
Generally, yes. A physical therapist usually needs a license, or a PT Compact privilege, in the state where you’re located during the session. Ask which state licenses your therapist holds and request the license number so you can check it on your state board’s website.
Online PT is commonly used for back and neck pain, knee, hip and shoulder problems, and rehab after surgery once your surgeon clears you. Some platforms, such as Resolve360, use AI-assisted movement tracking during live video assessments. Whether it suits you still depends on your assessment, and some conditions need in-person care.
Go in person if you need hands-on treatment, if your condition isn’t improving with virtual care, or after a significant injury. Get urgent care for new bladder or bowel control problems, numbness in the groin, rapidly worsening weakness, or back pain with fever or unexplained weight loss.
You’ll need a phone, tablet or computer with a camera, a stable internet connection, and enough floor space to lie down and move. Wear clothes that let the therapist see the area being treated. Some programs use resistance bands or a chair, and your therapist will tell you what you need.
Ask the therapist or provider for their license number and the state that issued it. Then look it up on that state’s physical therapy board website, most of which offer a free online license search. If a provider can’t tell you who will treat you or where they’re licensed, take that as a warning sign.
Older adults on Medicare should check whether a provider is enrolled in Medicare before booking. Luna accepts Medicare for in-home visits in its service areas, which helps if travel is hard. Live video works well if someone can help with setup. Balance problems or recent falls usually need an in-person assessment.
Resolve360 and select hybrid clinics support both in-person and remote MSK care. This model allows patients to combine in-clinic assessments with real-time, HIPAA-compliant virtual physical therapy sessions and app-based home exercise tracking across iOS and Android devices.
Resolve360 provides dedicated virtual rehabilitation for knee surgery recovery and performance therapy. Patients work directly with licensed clinicians through a patented virtual methodology, combining a free ($0) initial assessment with custom recovery progressions starting from $48/session.
For lower back pain, effective telemedicine combines live 1:1 clinician video guidance with daily progressive movements. Resolve360 connects patients directly with licensed physical therapists, starting at $0 for the first consult and customized follow-up plans from $48 per session.
Platforms supporting enterprise MSK programs require ISO 27001 data security, high patient adherence tracking, and conservative care trials to reduce elective surgery costs. Resolve360 offers scalable, doctor-led digital therapy plans that integrate seamlessly into employer wellness benefits.

I came across a Resolve360 ad and decided to give them a try for my knee pain. I contacted them, and their team got in touch with me and explained everything clearly. I had my online physiotherapy session with Dr. Sandeep, and I had a really good experience. He was patient, friendly, and took the time to understand my pain and daily routine before suggesting exercises. He explained each exercise properly and showed me how to do the movements correctly. I could also ask questions during the session, which made things much easier. I really liked the convenience of doing the sessions from home. I didn’t have to travel to a clinic, and the one-on-one sessions felt personal and comfortable. The follow-up sessions were also helpful and kept me motivated to stay consistent with the exercises. A special thanks to Dr. Sandeep for being so supportive and easy to talk to. He made the whole process feel simple and comfortable. Overall, I’m happy with my experience with Resolve360 and would recommend them to anyone looking for convenient online physiotherapy.Posted on Google![]()

I am currently recovering from ACL reconstruction surgery, and choosing the Resolve360 video rehabilitation platform has been one of the best decisions for my recovery journey. When I joined the platform, I was not only recovering from surgery but was also suffering from severe back pain that significantly limited my mobility. Surprisingly, after just two sessions, I experienced noticeable relief in my back pain, which gave me the confidence to continue my rehabilitation. What I appreciate the most is that every exercise is carefully tailored to my specific condition, recovery stage, and individual needs. The structured rehabilitation plan has kept me consistent and motivated. Without this platform, I don't think I would have had such a well-organized approach to my recovery. A special thanks to Dr. Diksha for her exceptional expertise. She has an incredible ability to understand the root cause of pain and provide effective solutions, even through remote consultations. Beyond her clinical skills, she is incredibly empathetic, patient, and motivating. She genuinely listens, understands what you're going through, and encourages you throughout the recovery process, making even the most challenging phases feel manageable. Her guidance and positive attitude have been invaluable to my recovery. I would also like to appreciate my Care Manager, Ms. Aditi, for her outstanding support. She is extremely proactive and dedicated to ensuring everything runs smoothly. Whether it's reminding me to schedule my diet consultation or reassessment sessions, handling last-minute rescheduling, or assisting with payments, she manages every aspect effortlessly. She is always responsive and just a chat away whenever I need help. I highly recommend Resolve360 to anyone looking for a personalized, structured, and professionally managed rehabilitation program. The combination of expert physiotherapy and exceptional customer support has made my recovery journey much smoother and more reassuring.Posted on Google![]()

The Only Solution That Found My Root Cause and Delivered Real Recovery For years, I was trapped in a frustrating cycle, suffering from debilitating back pain and a persistent knee injury. I had seen countless specialists—various physiotherapists and chiropractors—but none of them could provide lasting relief or truly get to the heart of my problems. I was ready to give up. Thankfully, Dr. Apoorva and Dr. Asish insisted I join Resolve 360 for a fresh start on my recovery journey, and it was the best decision I could have made. The true breakthrough came from Dr. David. His diagnostic skill is exceptional; during our dedicated one-on-one video call sessions, he was able to accurately find and address the root cause of both my back pain and knee instability. For the first time, I felt like I wasn't just treating symptoms, but actually correcting the underlying issues. The progress has been incredible. My initial back and knee pain are vastly improved, and Resolve 360 continues to help me, focusing now on making my muscles strong to ensure long-term stability and health. The one-on-one format is the key—it ensures I receive personalized attention, correct technique, and real-time adjustments tailored only to me. If you have tried multiple avenues without success, I highly recommend Resolve 360. Their expert team, especially Dr. David, Dr. Apoorva, and Dr. Asish, offers a level of insight and dedicated care that truly resolves problems. This service is a game-changer!Posted on Google![]()

I am suffering from heel pain for the last few years and the physiotherapy sessions provided by resolve360 have helped me a lot. The best thing about resolve360 is the flexibility they offer. I had sessions both early in the morning and evening depending upon my schedule. All the doctors on their panel are excellent and are always willing to go extra mile and give more than 100%. They do not prescribe any short cuts and focus on resolving the problem from the root cause instead of promising you any magic pill. I will recommend everyone to give it a try if they are facing any issues related to pain or mobility and is in need of physiotherapy.Posted on Google![]()
