Understanding Your Options: Live Video vs. App-Guided Virtual PT

Companies use “telehealth,” “virtual,” “remote” and “digital” physical therapy loosely, so the label alone will not tell you what kind of care you are buying. These are the four main models.

Live video physical therapy (telehealth or virtual PT)

Real-time, one-on-one virtual PT with a physical therapist who assesses you, guides exercises and adjusts your plan. A personalized home exercise program usually fills the time between visits. See what happens in a live online PT session.

Employer-sponsored digital MSK programs

Virtual musculoskeletal care that employers and health plans buy for members, such as Hinge Health, Sword Health and Omada. Members usually pay nothing directly, but only eligible people can enroll.

App-guided exercise (digital physical therapy)

Exercise programs led by an app, sometimes with motion sensors or AI feedback. A clinician reviews your progress, but you may not see a therapist live every time. Clinics may bill app tracking between visits as remote therapeutic monitoring.

In-home and hybrid physical therapy services

A therapist treats you in person at home, sometimes combined with video visits. Useful when you need hands-on care.

The 3 Main Categories of Virtual Physical Therapy Companies

Virtual physical therapy companies in the USA generally fall into three distinct care delivery models, depending on who covers the cost and how you access treatment:

TYPE 1

Employer and health-plan programs

Hinge Health, Sword Health, Omada, Vori Health and RecoveryOne. Usually no direct cost if your employer or plan offers them, but you cannot simply sign up if it does not.

TYPE 2

Direct-booking virtual PT

Resolve360, Agile Virtual PT and ATI Physical Therapy's online service. Anyone can book, subject to provider availability and eligibility. Payment is self-pay or insurance, depending on the provider.

TYPE 3

Hybrid in-home providers

Luna and Kins. A therapist treats you at home, and Kins offers virtual sessions in some states. A potential fit if you need hands-on care.

The top 10 telehealth physical therapy companies in the USA

Virtual physical therapy companies in the USA generally fall into three distinct care delivery models, depending on who covers the cost and how you access treatment:

1. Resolve360: Global Direct-Access Virtual Care Platform for live 1:1 virtual PT

Resolve360 is a premium Global Direct-Access Virtual Care Platform. US patients book expert, consultative virtual physical therapy directly, with no employer plan, insurance approval, referral or waitlist.

  • Experience: 100% in-house physical therapists and MSK experts with 10–30+ years of clinical experience.
  • Scale: 50,000+ patients treated globally and 2 million+ sessions completed.
  • Patient rating: 4.9 average.
  • Quality and security: ISO 9001:2015 quality-managed processes; ISO/IEC 27001 secure, HIPAA-compliant care.
  • Methodology: clinical methodology protected by Government of India Patent No. 596041.

Start with a $0 Free Root Cause Analysis: a 30-minute one-on-one live video consultation with a senior physical therapist who identifies the root cause of your pain and maps your next step. Exact pricing depends on your specific condition and clinical severity.

Customized plans start from $48 per session. The US page also lists a one-week plan at $88 and a four-week plan at $258. Exact pricing depends on your specific condition and clinical severity.

Best for: lower back pain, sciatica, neck and shoulder pain, knee pain and rehab after knee surgery, frozen shoulder and tennis elbow. Book your $0 Free Root Cause Analysis.

2. Hinge Health: employer-sponsored digital MSK care

Hinge Health is one of the largest US digital musculoskeletal companies and is publicly traded on the NYSE. Its program combines app-based exercise therapy with a care team. In June 2025 it announced HingeSelect, an in-person provider network that it says offers care at up to 50% below PPO rates. It reported $213 million in revenue for Q2 2026.

Consider it if your employer or health plan offers it. Keep in mind that you generally cannot enroll on your own. If you are not eligible, see our guide to Hinge Health alternatives.

3. Sword Health: AI Care for musculoskeletal pain

Sword Health runs what it calls an AI Care platform for pain treated at home. On January 28, 2026, it announced the $285 million acquisition of Kaia Health and said it will replace Kaia’s US musculoskeletal program with its own. Sword reports more than 700,000 members and over 1,000 enterprise clients.

Consider it if your employer or plan offers Sword, or you were a Kaia member. Keep in mind that access usually depends on a sponsor, so ask how much live clinician contact your program includes.

4. Omada Joint & Muscle Health: a dedicated physical therapist plus an app

Omada’s musculoskeletal program starts with a video visit with a physical therapist, who builds a treatment plan in the Omada app. Members in the treatment program are matched with a dedicated, licensed physical therapist, and Omada says the first appointment can happen as early as 24 hours after enrollment. A free exercise kit ships to members. A preventive version without a physical therapist is offered to members without a current injury.

Consider it if you want regular contact with one physical therapist and your organization offers Omada. Use its eligibility checker to confirm.

5. Vori Health: physician-led virtual MSK care

Vori Health’s care team includes board-certified specialty physicians, physical therapists, registered dietitians and health coaches. Services include physical therapy, prescriptions, and imaging and lab orders, with in-person orthopedic care available. Access has come through partnerships with health plans and benefit administrators, such as small-business group plans.

Consider it if you may need a physician’s evaluation or imaging alongside physical therapy, and your plan includes Vori.

6. RecoveryOne: digital physical therapy through health plans

RecoveryOne offers a digital musculoskeletal physical therapy program, mainly through health plans and employers. It is now owned by TailorCare, a specialty musculoskeletal care company.

Consider it if your health plan lists RecoveryOne as a benefit. Ask your plan how enrollment works.

7. ATI Physical Therapy: a national clinic chain with online PT

ATI Physical Therapy is a traditional outpatient provider with more than 850 locations in 24 states, according to its April 2025 announcement. It offers online physical therapy through its CONNECT platform, hybrid care that mixes clinic and virtual visits, and remote therapeutic monitoring.

Consider it if you want the option to switch between virtual and in-clinic visits. Check whether your insurance covers ATI’s virtual visits.

8. Agile Virtual PT: virtual sessions from a clinic network

Agile Virtual PT is presented on the Alliance Physical Therapy Partners website. It describes a HIPAA-compliant platform where patients meet their therapist from home, and it works with clinics and employers. Its page did not list prices or accepted insurance when we checked.

Consider it if you want virtual PT backed by an in-person clinic network. Ask for current rates and whether a therapist is authorized in your state.

9. Luna: in-home physical therapy

Luna sends a physical therapist to your home rather than treating you by video. Luna says it accepts most major insurance plans, including Medicare and Medicare Advantage, and lists a self-pay rate of $135 per visit. We include it because many people compare it with telehealth options when they need hands-on care.

Consider it if you need manual therapy or have trouble traveling to a clinic.

10. Kins: in-home PT with virtual sessions in some states

Kins provides outpatient physical therapy at home, and its 2026 job listings show virtual roles, including men’s pelvic health, for therapists licensed in states such as Massachusetts, Virginia and Maryland. Service areas are limited, so check your location first.

Consider it if you live in a Kins service area and want a mix of home visits and video sessions.

How we chose these companies

We started from the companies people search for when they look up telehealth, virtual, remote and digital physical therapy companies, then checked each one’s website and recent announcements in October 2026. We compared:

  • Who can enroll: anyone, or only members of an employer or health plan
  • How much live contact you get with a licensed physical therapist
  • Care model: live video, app-based, AI-guided, in-home or hybrid
  • Whether prices are published
  • Insurance and Medicare options
  • Recent ownership changes that affect members

We removed three providers from the previous version of this page (PT Genie, Benchmark PT telehealth and Reflexion Health’s VERA) because we could not confirm a current telehealth physical therapy service that patients can use. No provider paid to appear here. If you are choosing a service for yourself rather than comparing companies, read our best online physical therapy services guide.

Employer-Sponsored Digital MSK Companies: Hinge, Sword, Omada and More

Hinge Health, Sword Health, Omada, Vori Health and RecoveryOne sell virtual musculoskeletal care to employers and health plans. Members usually pay nothing directly, but only eligible employees or plan members can enroll.

Recent changes in the market

  • Sword Health acquired Kaia Health for $285 million (announced January 28, 2026).
  • RecoveryOne is now part of TailorCare.
  • Hinge Health (NYSE: HNGE) launched its HingeSelect in-person network in 2025 and agreed in August 2026 to acquire Cylinder Health.

What employers compare when choosing a vendor

  1. Clinical model: how often members speak live with a physical therapist, and what the app or AI handles.
  2. Escalation: referral paths to in-person care, physicians or imaging.
  3. Engagement: who can enroll and what share of members stay active.
  4. Outcomes reporting: pain, function and cost measures, and how often they are shared.
  5. Pricing and privacy: fee structure, HIPAA terms and data sharing.

No employer benefit? Direct-access companies such as Resolve360 do not require an employer plan. See our guide to Hinge Health alternatives.

Pricing Guide: How Much Does Virtual Physical Therapy Cost?

COMPANY HOW YOU PAY PUBLISHED STARTING COST
Resolve360 Out-of-pocket, direct booking $0 Free Root Cause Analysis; plans from $48/session*
Luna Insurance (including Medicare) or self-pay $135 per in-home visit
Hinge, Sword, Omada, Vori, RecoveryOne Employer or health plan Paid by the sponsor

*Exact pricing depends on your specific condition and clinical severity. See current Resolve360 plans.

Medicare telehealth flexibilities for physical therapists run through December 31, 2027. For a full breakdown of out-of-pocket costs, Medicare, private insurance and HSA/FSA, read our online physical therapy cost and insurance guide.

Direct Access: Do You Need a Doctor’s Referral for Virtual PT?

All 50 states allow some form of direct access to physical therapy without a physician referral, according to the APTA, though details vary by state and payer. The Federation of State Boards of Physical Therapy says a physical therapist must be authorized to practice in the state where the patient is located, usually through a state license or a PT Compact privilege. As of September 2026, 37 states and Washington, D.C. were issuing compact privileges.

Clinical Evidence: Is Virtual Physical Therapy Effective for Pain Relief?

For many musculoskeletal conditions, the evidence is encouraging. A systematic review and meta-analysis by Cottrell and colleagues in Clinical Rehabilitation (2017) found that real-time telerehabilitation was effective and comparable to standard care for improving physical function and pain. Results depend on the condition and on doing the exercises between sessions. Read more about whether online physical therapy can help with sciatica.

How to choose a telehealth physical therapy company

  1. Check your benefits first. Ask HR or your insurer whether you have Hinge Health, Sword, Omada, Vori Health or RecoveryOne.
  2. Decide how much live contact you want. Live video every session and app-guided programs feel very different.
  3. Look for expert experience. Ask how many years of clinical experience your physical therapist has.
  4. Compare the real starting cost. A $0 Free Root Cause Analysis lets you judge fit before you pay; a first visit elsewhere can cost $135. Exact pricing depends on your specific condition and clinical severity.
  5. Ask about escalation. Find out what happens if you need imaging, a physician or in-person care.
  6. Check security. Look for HIPAA compliance and ISO/IEC 27001 security protocols.

Final Verdict: Which Telehealth PT Company is Right for You?

If your employer offers Hinge Health, Sword Health or Omada, check your eligibility. If you want a physical therapist on live video every session, with no employer plan or referral, start with Resolve360’s $0 Free Root Cause Analysis. Choose in-home care such as Luna when you need hands-on treatment.

Frequently asked questions

The top telehealth physical therapy companies in the USA are Resolve360, Hinge Health, Sword Health, Omada, Vori Health, RecoveryOne, ATI Physical Therapy, Agile Virtual PT, Luna and Kins. They differ by care model: live 1:1 video, app-guided digital MSK programs, or in-home and hybrid physical therapy.

Resolve360, ATI Physical Therapy, Agile Virtual PT, Luna and Kins accept direct patient bookings. Hinge Health, Sword Health, Omada, Vori Health and RecoveryOne are offered mainly through employers and health plans, so individuals usually cannot sign up for them on their own.

Yes. Sword Health, Omada’s Joint & Muscle Health program, Vori Health and RecoveryOne offer similar employer-sponsored digital musculoskeletal care. Each uses a different model, from AI-guided exercise to physician-led care teams. For people without employer coverage, direct-access companies such as Resolve360 offer live 1:1 virtual physical therapy.

Direct-access companies such as Resolve360, Agile Virtual PT and ATI Physical Therapy are the best alternatives to Hinge Health and Sword Health without employer coverage. Resolve360 offers live 1:1 video every session, a $0 Free Root Cause Analysis, and plans from $48 per session. Exact pricing depends on your specific condition and clinical severity.

Both sell digital musculoskeletal programs to employers and health plans. Hinge Health pairs app-based exercise therapy with a care team and added HingeSelect, an in-person provider network, in 2025. Sword Health runs an AI Care platform and bought Kaia Health in January 2026. Your employer usually decides which one you can use.

Sword Health acquired Kaia Health for $285 million, announced January 28, 2026, and is replacing Kaia’s US musculoskeletal program with its own. RecoveryOne is now part of TailorCare. Hinge Health, publicly traded on the NYSE, agreed in August 2026 to acquire Cylinder Health, a digestive health company.

Telehealth physical therapy companies deliver live video sessions with a physical therapist who adjusts your plan in real time. Digital physical therapy companies usually run app-guided exercise programs, sometimes with sensors or AI feedback, where a clinician reviews progress but may not see you live every session.

Some do, and the level of contact varies. Omada matches members in its treatment program with a dedicated licensed physical therapist, and Vori Health uses physicians and physical therapists. Other programs rely more on app-guided or AI-guided exercise. Resolve360 includes a live physical therapist in every session.

Luna and Kins offer in-home physical therapy, where a therapist treats you at home. Kins also offers virtual sessions in some states, and ATI Physical Therapy combines online physical therapy with more than 850 clinics. Luna accepts most major insurance, including Medicare, and lists $135 per self-pay visit.

Prices vary by company and access model. Resolve360 starts with a $0 Free Root Cause Analysis, with plans from $48 per session; exact pricing depends on your specific condition and clinical severity. Luna lists $135 per in-home visit, and employer programs such as Hinge Health are paid by the employer or plan.

The $0 Free Root Cause Analysis is Resolve360’s free initial assessment: a 30-minute one-on-one live video consultation with a senior physical therapist who identifies the likely root cause of your pain and recommends your next step. Exact pricing depends on your specific condition and clinical severity.

Employers usually compare the clinical model, how often members see a physical therapist live, enrollment and engagement rates, in-person referral options, outcomes reporting, pricing structure and data privacy terms. Ask each vendor for eligibility rules, sample outcome reports, and how its program connects with your health plan.

Yes. Hinge Health trades on the New York Stock Exchange under the ticker HNGE. It reported $213 million in revenue for the second quarter of 2026, up 53% year over year, and sells its digital musculoskeletal program mainly to employers and health plans rather than individual patients.

Resolve360 is a Global Direct-Access Virtual Care Platform with 100% in-house physical therapists who have 10–30+ years of clinical experience. It has treated 50,000+ patients globally, completed 2 million+ sessions and holds a 4.9 average patient rating. Its processes are ISO 9001:2015 quality-managed and ISO/IEC 27001 secure.

If Resolve360’s experts determine your condition requires mandatory in-person care, such as severe acute trauma or complete paralysis, they will transparently advise you to seek local in-person care immediately. Patient safety comes first, so the Root Cause Analysis screens for warning signs before any virtual treatment plan begins.

I came across a Resolve360 ad and decided to give them a try for my knee pain. I contacted them, and their team got in touch with me and explained everything clearly. I had my online physiotherapy session with Dr. Sandeep, and I had a really good experience. He was patient, friendly, and took the time to understand my pain and daily routine before suggesting exercises. He explained each exercise properly and showed me how to do the movements correctly. I could also ask questions during the session, which made things much easier. I really liked the convenience of doing the sessions from home. I didn’t have to travel to a clinic, and the one-on-one sessions felt personal and comfortable. The follow-up sessions were also helpful and kept me motivated to stay consistent with the exercises. A special thanks to Dr. Sandeep for being so supportive and easy to talk to. He made the whole process feel simple and comfortable. Overall, I’m happy with my experience with Resolve360 and would recommend them to anyone looking for convenient online physiotherapy.

I am currently recovering from ACL reconstruction surgery, and choosing the Resolve360 video rehabilitation platform has been one of the best decisions for my recovery journey. When I joined the platform, I was not only recovering from surgery but was also suffering from severe back pain that significantly limited my mobility. Surprisingly, after just two sessions, I experienced noticeable relief in my back pain, which gave me the confidence to continue my rehabilitation. What I appreciate the most is that every exercise is carefully tailored to my specific condition, recovery stage, and individual needs. The structured rehabilitation plan has kept me consistent and motivated. Without this platform, I don't think I would have had such a well-organized approach to my recovery. A special thanks to Dr. Diksha for her exceptional expertise. She has an incredible ability to understand the root cause of pain and provide effective solutions, even through remote consultations. Beyond her clinical skills, she is incredibly empathetic, patient, and motivating. She genuinely listens, understands what you're going through, and encourages you throughout the recovery process, making even the most challenging phases feel manageable. Her guidance and positive attitude have been invaluable to my recovery. I would also like to appreciate my Care Manager, Ms. Aditi, for her outstanding support. She is extremely proactive and dedicated to ensuring everything runs smoothly. Whether it's reminding me to schedule my diet consultation or reassessment sessions, handling last-minute rescheduling, or assisting with payments, she manages every aspect effortlessly. She is always responsive and just a chat away whenever I need help. I highly recommend Resolve360 to anyone looking for a personalized, structured, and professionally managed rehabilitation program. The combination of expert physiotherapy and exceptional customer support has made my recovery journey much smoother and more reassuring.

The Only Solution That Found My Root Cause and Delivered Real Recovery For years, I was trapped in a frustrating cycle, suffering from debilitating back pain and a persistent knee injury. I had seen countless specialists—various physiotherapists and chiropractors—but none of them could provide lasting relief or truly get to the heart of my problems. I was ready to give up. Thankfully, Dr. Apoorva and Dr. Asish insisted I join Resolve 360 for a fresh start on my recovery journey, and it was the best decision I could have made. The true breakthrough came from Dr. David. His diagnostic skill is exceptional; during our dedicated one-on-one video call sessions, he was able to accurately find and address the root cause of both my back pain and knee instability. For the first time, I felt like I wasn't just treating symptoms, but actually correcting the underlying issues. The progress has been incredible. My initial back and knee pain are vastly improved, and Resolve 360 continues to help me, focusing now on making my muscles strong to ensure long-term stability and health. The one-on-one format is the key—it ensures I receive personalized attention, correct technique, and real-time adjustments tailored only to me. If you have tried multiple avenues without success, I highly recommend Resolve 360. Their expert team, especially Dr. David, Dr. Apoorva, and Dr. Asish, offers a level of insight and dedicated care that truly resolves problems. This service is a game-changer!

I am suffering from heel pain for the last few years and the physiotherapy sessions provided by resolve360 have helped me a lot. The best thing about resolve360 is the flexibility they offer. I had sessions both early in the morning and evening depending upon my schedule. All the doctors on their panel are excellent and are always willing to go extra mile and give more than 100%. They do not prescribe any short cuts and focus on resolving the problem from the root cause instead of promising you any magic pill. I will recommend everyone to give it a try if they are facing any issues related to pain or mobility and is in need of physiotherapy.
