What is online physical therapy?

Online physical therapy, also called virtual physical therapy or telehealth physical therapy, is care from a physical therapist delivered through video instead of a clinic visit. The PT evaluates how you move, explains what is likely contributing to your symptoms, and coaches you through exercises in real time.

 

There are two main formats:

  • Live video sessions. You and your PT are on camera at the same time. This format is closest to a clinic visit and is the focus of this guide.
  • App-based programs. You follow recorded exercises in an app, sometimes with motion sensors or messaging, and a clinician reviews your progress. These are often offered through employers or health plans.

 

Many providers combine the two, using live visits for assessment and progress checks and an app for exercises between visits.

How online physical therapy works, step by step

  1. Book and complete intake. You share your main problem, how long you have had it, past injuries or surgeries, medications, and goals. You can share imaging reports if you have them, but you usually don’t need them to start.
  2. Safety screening. Your PT checks for warning signs that need a doctor or emergency care first. If online care is not right for you, a good provider will say so.
  3. First video assessment. Your PT takes a detailed history and asks you to do specific movements on camera. The next section explains what they look for.
  4. A personalized plan. You agree on goals, how often to meet, and a home exercise program that fits your symptoms, schedule, and space.
  5. Live treatment sessions. Your PT demonstrates exercises, watches you do them, corrects your form, and makes them harder as you improve.
  6. Practice between visits. Most progress comes from doing your exercises consistently. Many providers send exercise videos or written instructions and let you message questions.
  7. Reassessment, discharge, or referral. Your PT retests your movement and function, updates the plan, and refers you for in-person care, imaging, or a physician visit if progress stalls or new symptoms appear.

What happens during an online physical therapy consultation?

The first online physical therapy consultation is usually the longest visit. It starts with questions: where it hurts, what makes it better or worse, how it affects your sleep and work, and what you have already tried.

 

Your PT then watches you on camera. Depending on your problem, they may ask you to:

 

  • Walk toward and away from the camera
  • Stand up from a chair without using your hands
  • Do a small squat, step onto a stair, or balance on one leg
  • Raise your arms forward and to the side so both sides can be compared
  • Bend, twist, or reach to see which movements bring on symptoms
  • Rate your pain and fill out a short questionnaire about daily function

 

Your PT uses these findings to form a working assessment and explain it in plain terms. Video has limits: a PT can’t feel a joint, test muscle strength by hand, or perform manual therapy through a screen. If those tests matter for your condition, your PT should tell you and suggest an in-person visit.

What you need for a virtual physical therapy session

  • A device with a camera and microphone. A laptop or tablet is easier to see than a phone.
  • A stable internet connection. Run a short test call before your first visit.
  • Room to move. Enough clear floor to lie down and take a few steps safely.
  • Camera placement. Prop the device at about waist height, far enough away that your whole body is in view. Your PT may ask you to turn sideways.
  • Light in front of you, not behind you, so your movements are visible.
  • Clothing that shows the area being assessed, such as shorts for a knee problem.
  • A sturdy chair, a wall or counter for support, and a towel or mat.
  • A private space, and another adult nearby if you are at high risk of falling.

Benefits of online physical therapy

  • No travel. You skip the commute and waiting room, which helps if driving is painful, transportation is hard to arrange, or your schedule is tight.
  • Flexible scheduling. Some providers offer early-morning, evening, or weekend times.
  • Access from rural areas. You can see a PT even if the nearest clinic is far away, as long as the PT can practice in your state.
  • Care in your real setting. Your PT can see your desk, stairs, or the chair you find hard to get out of and tailor advice to them.
  • Self-management skills. Because you do the exercises yourself, you learn how to handle flare-ups without clinic equipment.

Does online physical therapy work?

For some musculoskeletal conditions, research suggests that therapist-led telerehabilitation can work about as well as in-person care. The evidence is promising but not conclusive:

 

  • A 2017 systematic review and meta-analysis by Cottrell and colleagues found that real-time telerehabilitation improved physical function and pain across several musculoskeletal conditions and was comparable to usual in-person care.
  • A 2021 overview of 53 systematic reviews by Seron and colleagues, published in Physical Therapy, concluded that telerehabilitation could be comparable to in-person rehabilitation for conditions including osteoarthritis, low back pain, and hip and knee replacement. The authors also reported mixed results and called for better-quality studies.
  • A 2023 meta-analysis of randomized trials by Krzyzaniak and colleagues found that function, quality of life, and satisfaction were similar between telehealth and face-to-face care right after treatment.

 

These results don’t apply to every person or condition, and they don’t guarantee a particular outcome. Your assessment decides whether online care is suitable for you.

Limitations and when to choose in-person care

Online physical therapy has real limits:

 

  • No manual therapy or other hands-on treatment
  • Some physical exam tests can’t be done reliably on video
  • Results depend on your camera, connection, and space
  • It is not meant for emergencies or rapidly worsening symptoms

Get urgent medical care instead of booking online if you have:

  • New weakness in an arm or leg, or new numbness in the groin or inner thighs
  • New loss of bladder or bowel control
  • Chest pain or shortness of breath
  • A swollen, warm, or painful calf, especially after surgery or a long period of inactivity
  • Back pain with fever, or after a serious fall or accident
  • A suspected broken bone or dislocated joint, or sudden severe pain

In an emergency, call 911. This list is not complete. If you are unsure, contact a doctor.

In-person or hybrid care may also be a better choice if your symptoms are unclear, you aren’t improving as expected, you are at high risk of falling, you need early post-surgery wound or brace checks, or your PT recommends it. Being referred for in-person care is a normal part of good online care.

How much does online physical therapy cost?

The cost of online physical therapy depends on whether you pay out of pocket or use insurance, the provider, and how many visits you need.

  • Without insurance: A 2023 GoodRx Health guide put typical self-pay sessions, in person or online, at about $75 to $150. Prices vary widely by provider and location, and a first evaluation often costs more than a follow-up visit.
  • With private insurance: You usually pay a copay or coinsurance, and your deductible may apply. Coverage for video visits, visit limits, and prior authorization rules vary by plan.
  • Medicare: The Consolidated Appropriations Act, 2026 extended Medicare telehealth eligibility for physical therapists through December 31, 2027. Medicare pays only for services from providers that bill Medicare, and Part B cost-sharing applies. This extension is temporary.
  • Medicaid: Coverage for telehealth physical therapy varies by state.
  • HSA or FSA: IRS Publication 502 lets you count therapy received as medical treatment as a medical expense. Your plan administrator may ask for a letter of medical necessity.

  • Does my plan cover physical therapy by video, and at the same cost as in-person visits?
  • Is this provider in network? If not, can I submit a superbill (an itemized receipt) for out-of-network reimbursement?
  • Do I need a referral or prior authorization?
  • How many physical therapy visits does my plan cover each year?

How to choose an online physical therapy provider

  • State license. Ask which state licenses your PT holds. They must be licensed in the state where you are during the visit, or hold a PT Compact privilege for that state. You can check a license on your state physical therapy board’s website.
  • Who treats you. Confirm you will see a licensed PT, or a licensed physical therapist assistant supervised by a PT, rather than a coach or trainer.
  • Live or app-based. Decide whether you want real-time coaching, a self-guided program, or both.
  • An assessment first. Avoid services that send a generic exercise list without evaluating you.
  • Clear pricing and payment options, including whether they bill insurance, provide superbills, or accept HSA and FSA cards.
  • Privacy. Ask how your video sessions, messages, and records are protected.
  • A plan for in-person care. Ask what happens if you need a hands-on exam, imaging, or a physician.
  • Scheduling and continuity. Check time zones, availability, and whether you will see the same PT each time.

In most cases you don’t need a doctor’s referral. According to the American Physical Therapy Association (APTA), all 50 states allow some form of direct access to physical therapists, but many limit the number of visits or days before a referral is required. Your insurance plan may also have its own referral rules.
