Many people think their body recovers when they sleep. What if you are sleeping in the wrong position? Poor arm position, side sleeping, and insufficient support are responsible for irritating the sensitive tissues. If your shoulder pain from sleeping on side persists after waking up, it's time to check your sleeping position and identify the underlying cause with physiotherapy.
Do you know that your favorite sleeping position could be a reason behind your side sleeper shoulder pain?
When you spend 6-8 hours in the same position while sleeping, it can cause continuous pressure on the tendon, shoulder joint, and surrounding tissues. Don’t think your sleeping position is the only reason for the stiffness, but it can be a major contributor to side sleeping shoulder pain.
A licensed physiotherapist understands your problem and identifies the root cause. With targeted treatment, exercises, and posture correction, you can sleep more comfortably.
The shoulder is known to be the most mobile joint in your body. Shoulder movement generally depends on tendons, muscles, ligaments, and other soft tissues. When you sleep in one position for prolonged hours, it can result in:
The underlying medical conditions like bursitis, rotator cuff irritation, or early arthritis can worsen the stiffness and night discomfort.
If you are looking for relief from this problem, then you should contact Resolve360, a trusted online platform to access the best frozen shoulder physiotherapy. Our physiotherapists connect with you through video sessions and guide you while performing the exercises.
Your sleeping position is not a habit, as other factors also silently affect how your body rests. Whether it’s your body posture during the day, mattress support, or your stress level, they can disturb your pressure points and shoulder alignment.
Here are some common factors that can cause shoulder pain from sleeping on side.
If you are sleeping on the affected shoulder, it puts weight on joints and compresses rotator cuff tendons, subacromial bursa, and shoulder capsule.
A licensed physiotherapist works by recognizing the real cause and prescribing targeted exercises, manual therapy, and sleeping adjustments to lessen pressure on the shoulder.
Do you also like to sleep keeping one arm under the pillow and the other one above the head? This might feel relaxed, but unknowingly it can put pressure on the rotator cuff and shoulder capsule for prolonged hours.
Treatment is emphasized mainly on reinstating shoulder mobility, decreasing tendon irritation, and improving shoulder mechanics.
Like others, do you also think that a pillow is just for supporting your neck? Well, choosing the right pillow is also associated with shoulder alignment. If your pillow is too high or low, it can affect your shoulder positioning and put additional stress on muscles and joints.
A professional physiotherapist can suggest the right pillow height and sleeping alterations depending on your body type and symptoms.
Among others, rotator cuff tendinopathy is one of the common reasons behind the shoulder pain from sleeping on side at night. The change in blood flow whenever you lie down increases pressure on the tendon and makes symptoms worse.
Take advantage of the evidence-based physiotherapy that generally includes strengthening exercises, manual therapy, activity modification, and progressive programs.
Due to the frozen shoulder, many people experience severe night-time pain. In whichever position they sleep, every position seems uncomfortable.
If you start early, frozen shoulder physiotherapy helps maintain movement, decrease stiffness, and improve overall function during each stage of frozen shoulder recovery.
By selecting the right sleeping position, it is possible to make a substantial difference in treating shoulder pain. The wrong sleeping position can directly impact muscle relaxation, joint pressure, and overnight recovery. That’s why it becomes important to understand which sleeping positions support your shoulder well and minimize symptoms.
This position helps reduce pressure on both shoulders and encourages stable shoulder alignment. Hence, it is suggested to use this position for rotator cuff injuries and frozen shoulder.
The position is ideal for improving sleep quality and decreasing the compression of a painful shoulder.
Do follow a tip of placing a pillow in front of your chest and allow your affected arm to rest on it. By doing this, there is reduced strain on the shoulder joint.

Yes. If you are sleeping on your side, especially when it’s your affected shoulder, you are putting unnecessary pressure on your shoulder joint. This results in stiffness, side sleeping shoulder pain, or worsening of your shoulder symptoms.
shoulder pain from sleeping on side gets worse at night because you sleep in one position only for several hours. This increases pressure on the shoulder joint. Medical conditions like rotator cuff tendinopathy, frozen shoulder, bursitis, and shoulder impingement are mainly responsible for causing night-time side sleeping shoulder pain that can disturb your sleep.
Most people prefer sleeping on their back and find it more comfortable, as it reduces pressure on the shoulder. In case you prefer side sleeping, it is preferable to sleep on your unaffected side and place a pillow under the painful arm as support.
Yes. If you keep a pillow too high or too low, it lacks support and disturbs the alignment of the neck, shoulders, and upper back. Due to poor posture, there is increased pressure on shoulder muscles and joints.
Contact the physiotherapists or medical professional if your shoulder pain sleeping on side continues for more than 1–2 weeks, regularly disturbs you at night, restricts your ability to move your arm, causes weakness, or continues to degrade even when you take rest.

Resolve360 is great platform for online physiotherapy.
I was suffering from scapula dyskinesia and had lot of pain in right shoulder movement. I was introduced to Resolve 360 by my oncologist.
Within a month (26 sessions) there was a considerable improvement. Sessions are very cost-effective and doctors are very kind and efficient.
Doctor David has been really great and helpful. I would highly recommend Resolve 360.
I took prenatal care program from Resolve 360. Dr. Preyrna was assigned to me and she has done commendable job. I did prenatal exercises under her supervision, she designed program according to my body need. With her help my pregnancy was healthy and I got good dilations at the time of delivery. I am blessed with a healthy baby girl and very happy with Resolve 360.
Best therapy sessions that too at home and timings are very flexible. I had a issue in my neck and lower back since almost 4 to 5 years, taken so many treatments and physio sessions. But all gave a temporary relief. Then my friend referred me this Resolve 360 in 2020, in starting i was hesitant as everything was online, but after taking almost 10 sessions i got much relief, and now i am almost free of pain. Thanks to the team of Resolve 360.
I had online Physiotherpy sessions by Prerana for my heel pain. ( Tendo Achilis Sprain).
Though virtual, it was more interactive.
She was very observant throughout the sessions, correcting me while doing the exercises and was assertive in her outlook.
I found her instructions and methodology very helpful. She is updated in her academic knowledge in the field of Physiotherpy. She clarified my doubts about the role of Laser and Ultrasound treatment quoting evidence based data. With my personal experience I agree with her opinion that these are more of placebo effect.
Physiotherpy though not a miracle treatment, is a compelling necessity to most of the musculoskeletal and neurological illnesses. It needs to be accepted as healthy way to live.
From being a couch potato with poor cardio respiratory system to reaching pre-athlete levels of fitness in a span of 3 months, my journey with Resolve360 and Dr.David (Physiotherapist) has been anything but magical!
About 3 to 4 months ago, my blood report showed very high bad cholesterol levels, I was low on stamina and had frequent breathing issues. My GP prescribed me cholesterol meds for a month but I felt that alone wouldn’t get me back in shape because I needed a major lifestyle overhaul.
Dr.David educated me about my condition in detail and where it could lead me to if I don’t take action in the long run. He put me on a balanced workout regime being easy with cardio workouts in the initial days and gradually upgraded it observing me closely. He also advises me on dietary choices and follows up frequently. During the initial days, my ears used to get blocked within 5 wall squats and I stop right away but now I do over 20 in one shot and hardly feel a thing. I used to lose balance and fall off over my face within 6 push-ups or 20 seconds of plank and now I do about 24 push-ups in one shot. My heart rate used to raise above 150 bpm doing jumping jacks or other cardio workout within 20-30 seconds. You won’t believe I don’t do any of these cardio workouts without additional weights on my feet these days and my heart rate and recovery are as steady as they could get. I couldn’t run for over 2 min without stopping and gasping and now I cover about 4.6 km in 30 min and follow-up with a 20 min walk. Not convinced yet? My VO2 Max (I use an Apple Watch to measure this stat) was just about 33 while I started with Resolve360 and David. That reflects a very poor cardio respiratory fitness for a 34 year old. It’s 43.5 today ! Easily above average. Please read upon about VO2 Max and how it directly relates to cardio respiratory fitness so you get an accurate picture about my monumental progress in the last 3 months. I’ve been trying to get into a regular physical workout and balanced diet discipline since my 20s but I’d never been so motivated as I’m now. Give these guys a try. After all, the best investment you could ever make is on your health !
