ACL reconstruction is a surgery usually done to replace a torn anterior cruciate (pronounced as KROO-she-ate) ligament (ACL) — a major ligament in our knee. ACL injuries are most commonly seen during sports involving sudden stops and direction changes — such as football, basketball, volleyball, etc.
A ligament is a strong band of tissue that attach one bone to another bone. In ACL reconstruction, the turned ligament is removed and replaced with a band of tissue that connects muscle to bone (tendon). The graft tendon is either taken from another part of the patient’s knee or from any of the deceased donors.
ACL reconstruction is an outpatient surgery that is performed by a doctor who is specialized in surgical procedures of the bones and joints (orthopedic surgeon).
The ACL — one of two ligaments that cross the middle of the knee — connects our thighbone to our shinbone and helps in the stabilization of our knee joint.
Most ACL injuries happen during different sports and fitness activities that can put stress on the knee:
Sudden change in direction during the activity
Pivoting with your foot firmly planted
Incorrect landing during a jump
Sudden stop
Direct blow to the knee
An athlete wants to continue in his/her sport, especially if the sport involves activities like jumping, cutting, or pivoting
There is a multiple ligament injury
There is a torn meniscus that requires immediate repair
The injury is causing trouble in day-to-day activities
You are young (though other factors, such as activity level and knee instability are to be given more priority than age)
Once the patient is recovered from anesthesia, he/she can go home the same day. Before the patient leaves, he/she has to practice walking with crutches, and the surgeon may advise wearing a knee brace or splint to help in the protection of the graft.
The doctor will give specific instructions on controlling pain and swelling after surgery. In general, it’s essential to keep the leg elevated, apply a cold wrap or ice on the knee, and rest as much as possible.
Medications that are required to help with pain relief include drugs such as acetaminophen (Tylenol), ibuprofen (Advil, Motrin IB, others), or naproxen sodium (Aleve). A doctor might prescribe stronger medications for this, like meloxicam (Mobic), tramadol (Ultram, Qdolo, ConZip), or oxycodone (OxyContin, Roxicodone, and others). If opioids are prescribed, they should be taken only for extreme pain as they have many side effects and a significant risk of addiction.
Follow the surgeon’s advice on when to ice the knee, how long to use crutches, and when it’s safe to bear weight on the knee. The patient will also be told when he/she can shower or when should change dressings on the wound, and how to manage post-surgery care.
Progressive physical therapy after ACL surgery helps to elevate muscle strength around the knee and improve flexibility. A physical therapist will teach how to do exercises that should be performed either with continued supervision or at home. Following the rehabilitation plan is important for quick healing and achieving the best possible results.
