Physiotherapy for ACL Tears is recommended by doctors because it helps in significantly decreasing the chances of any progression of the tear and even reduces post-surgery complications, in case surgery becomes inevitable due to larger tears. This is important because entering into surgery with better motion and strength is directly correlated to faster post-surgery recovery.

Though this depends on the tear severity, some people might even manage the injury without surgery with the help of prehab or physiotherapy for ACL tear before surgery if it improves strength and functioning significantly such that you stop facing difficulty in your quality of life.

What is an ACL Tear?

An ACL Tear occurs when the Anterior Cruciate Ligament is stretched beyond its physiological limits. This causes a partial or complete rupture of the collagen fibres that form the ligament.

Let us understand about the ACL first.

It is one of the major ligaments inside the knee which connects thighbone to shinbone (femur to tibia). It makes the knee stable by preventing excessive forward movement of the shinbone (tibia) in relation to the thighbone (femur). It is also important for rotational stability during jumping, running and all other movements in sports.

When the ACL tears, rapid swelling or hemarthrosis takes place due to the blood vessels within the ligament rupturing too. It bleeds into the knee joint and this injury causes the ligament to lose mechanism of function and leads to instability in the leg region.

The ACL Tear can be classified as:

  • Grade 1 – Mild Stretching
  • Grade 2 – Partial Tear
  • Grade 3 – Complete Rupture

The severity of the condition affects the symptoms and also determines prognosis treatment.

Common Symptoms of ACL Tear

If symptoms of an ACL Tear are detected early, it becomes easy to treat it. Some typical features of an ACL Tear include:

  • Sudden ‘pop’ or snapping sensation at the moment of injury.
  • Severe pain
  • Inability to do activities
  • Rapid swelling
  • Giving away of knees or instability
  • Difficulty in bearing weight
  • Loss of range of motion

Other associated injuries can commonly occur. These can be:

  1. Meniscus tear
  2. Collateral ligament injuries (MCL. LCL)
  3. Bone bruises

These symptoms can be varied and determine the course of action for the physician.

Mechanism of Injury or Factors Affecting an ACL TEAR

An ACL Tear can occur due to non-contact as well as contact forces during dynamic movements. It is a very common injury for athletes and out of the two types, non-contact forces are a more common causative agent.

The non-contact cases include:

  • Sudden deceleration or stopping
  • Jumping and landing awkwardly
  • Pivoting or cutting with planted foot
  • Rapid direction changes

Direct contact cases can include collision during sports.

These conditions can put high torsional or shear stress on the ACL ligament. Which can make it tear.

What Can Be the Treatment Options For an ACL Tear?

The treatment options can vary depending upon the severity of the ACL Tear. You should consult your healthcare provider and discuss the treatment plan according to the diagnosis.

You can opt for the following Treatments under the guidance of your doctor:

1. Initial First Aid

Immediately after the injury has occurred, you should follow the RICE protocol.

  • Rest
  • Ice
  • Compression
  • Elevation

Following this advice can reduce swelling and pain along with decreased chances of tear progression.

2. Non Surgical Management

The non-surgical management of an ACL Tear is appropriate mostly for mild sprains and partial or low grade tears. In older or less active individuals also it is commonly used as the primary treatment plan.

The non-surgical care include:

  • Bracing
  • Physiotherapy
  • Gradual return to activity after rest

It is important to keep in mind that a fully torn ACL cannot recover on its own due to poor blood supply.

3. Surgical Treatments

The surgical treatments of ACL Tear can include:

  • ACL Repair
  • ACL Reconstruction

Although both these treatment plans can be considered after consulting your doctor, an ACL reconstruction is still the gold standard for most full tear cases.

ACL Repair vs ACL Reconstruction

ACL repair and ACL reconstruction are two surgical treatment procedures opted for acute and complete tear cases respectively
ACL Repair ACL Reconstruction
Stitching of the torn ACL back together using an implant or brace. The torn ligament is removed or replaced with a graft from the patient’s own tendon.
Higher overall risk More durable and lower risk
Recovers in less time Longer time for the graft to heal to the bone
Used in acute tear cases Mostly in complete ACL tears for athletes

What is Prehab?

Prehab is short for prehabilitation which aims towards health and physical preparation before a planned approach towards circumstances like surgery, injury risks or sports activities.

The term prehab also means preventive rehabilitation in therapy and injury and includes targeted exercises and training that decrease the chance of future injuries and help the body move better and stronger.

Prehabilitation or Physiotherapy for ACL Tear before Surgery:

After your ACL (Anterior Cruciate Ligament) is torn and before you go for surgery, you can go for ACL Tear physiotherapy so that you can reduce pain and swelling and even improve post-surgery healing.

Advantages of physiotherapy for ACL Tear before surgery are:

  • Reduced pain and swelling
  • Increased range of motion
  • Strengthen the knee muscles
  • Improve proprioception (balance and control)
  • Maintain health and fitness before surgery
  • Enhance post-op healing

Research shows that 4-6 weeks of prehab before ACL reconstruction can improve early strength and motion after surgery and may increase the chance of returning to sport at your desired level.

Common Physiotherapy Exercises for ACL Tear Before Surgery

The following exercises are helpful in reducing pain and swelling after a torn ACL and improving post-operative complications.

1. Glute Bridges

Glute Bridges provide indirect support to the knee joints because weak hips and glutes can also cause knee pain.

How is it done?

  • Lie down on a mat with both knees bent and feet flat on the floor, hip width apart
  • Slowly lift the hips and press your lower back towards the floor
  • Hold for 5 to 10 seconds
  • Repeat 15 to 20 times

Key Benefits:

  • Strengthens hip and glute muscle
  • Reduces pressure on the knee joint

2. Calf Stretch

Calf stretch can improve flexibility and mobility because tight calves can restrict movements and cause pain in the legs.

How is it done?

  • Stand facing a wall, firmly plant your hands on it
  • Step one foot back, keep the back leg straight with heels down and toes forward
  • Lean into wall until you feel a stretch in your calves
  • Hold for 20 seconds then switch sides
  • Do 3 to 5 sets.

Key Benefits:

  • Safe exercise for osteoarthritis and arthritis
  • Reduces stiffness
  • Improves blood circulation

3. Hamstring Stretch

Hamstring stretch also improves flexibility and enhances mobilization because tight hamstrings can make daily activities like walking and bending very difficult tasks.

How is it done?

  • Sit on a chair with one leg extended
  • Keep the back straight and lean forward slightly
  • Hold this stretch for 15 to 20 seconds
  • Switch the sides
  • do 3 to 5 times per leg

Key Benefits:

  • Increased flexibility
  • Reduced pain

4. Straight Leg Raises

Straight leg raises are especially helpful when climbing stairs becomes painful by making leg muscles strong without putting much pressure on the knees.

How is it done?

  • Lie down flat on a bed with one knee bent
  • Keep the other leg straight and tighten the muscles
  • Lift this straight leg 6 to 8 inches off the bed
  • Hold this position for 5 seconds
  • Slowly lower it down
  • Repeat this process with another leg and avoid holding your breath
  • Do the whole exercise 8 to 10 times.

Key Benefits:

  • Makes knee movement easier
  • Relieves pain

5. Quad Sets

Quadriceps Sets help strengthen these muscles and give supporting and stable legs. The large muscles in the front region of the thigh are called quadriceps. It is crucial to keep these muscles healthy and strong for normal movements.

How is it done?

  • Lie flat on a bed with both legs completely relaxed
  • Put a rolled towel below one knee
  • Straighten that leg and contract the muscles
  • Hold and feel the contraction for 5 to 10 seconds
  • Release and do the same with another leg
  • Repeat 10 to 15 times daily.

Key Benefits:

  • Improves joint support
  • Reduces pain.

6. Swimming/Aquatic Therapy

Swimming or doing aquatic exercises improve overall fitness and health by reducing strain on the muscles.

How is it done?

  • Water based movements or swimming with gentle strokes without jerky movements.
  • Always warm up before entering the pool.

Key Benefits:

  • Reduces strain on body muscles
  • Enhances endurance and mobility
  • Improves posture and coordination

Prevention and safety tips:

Following precautions should be taken before starting physiotherapy exercises for acl tear before surgery:

  • Exercises should always be done on a firm and flat surface.
  • Sudden and jerky movements should be avoided.
  • Stop immediately if the pain starts to increase or radiate.
  • The exercises should be done under professional guidance

Consult a physiotherapist in cases of severe tears, fractures or recent surgery beforehand.

How Soon Should You Start Physiotherapy for ACL Tear Before Surgery?

You can start acl tear physiotherapy before surgery as soon as you start experiencing pain and swelling in the knees. You should consult your doctor before starting and during physiotherapy to do required and guided movements only so that it does not worsen your condition.

This is crucial because:

  • You can have better strength and functioning after surgery
  • Can start post-surgery rehab soon
  • May reduce post-surgery complications
  • Faster returning to sports

The best results come from a guided program with a physical therapist, tailored to your specific situation.
