Suffering from knee joint pain and getting knee replacement surgery is quite common among elderly people between the age group of 65 years and above. With growing age, bone density and strength keep on degrading, which can lead to several joint problems and one of them is knee joint pain. Knee joint pain can occur due to other secondary reasons too which will be discussed in the article. The article covers the care and precautions, that one should take pre-knee replacement surgery and post-knee replacement surgery.

What is Knee Replacement Surgery?

Knee replacement is a surgical procedure in which the knee cap (if required) and the damaged parts of the joints are removed and replaced by a prosthesis.

Knee replacement surgery is also known as knee arthroplasty and is effective for severely damaged knee joints which can make the person unable to move and walk properly.

What are the Causes of Knee Replacement Surgery?

With growing age, it is common that bone density starts degrading, it loses strength and certain elements like calcium that make it stronger.

Causes of Knee Replacement Surgery

Knee joint replacement can be done due to many reasons and the most common is arthritis in old people. Other causes include:

  1. Rheumatoid arthritis

  2. Osteoarthritis

  3. Osteonecrosis

  4. Haemophilia

  5. Gout and pseudogout

  6. Unusual bone growth

  7. Due to side effects of any previous injury.

  8. Tear of any ligament.

  9. Due to bone fractures in an earlier stage of life.

  10. Sitting in a constrained area.

  11. Sitting on knees for a prolonged period.

  12. Knee injury.

  13. Infections.

What can a patient expect

Before the Surgery

The orthopedic surgeon can do the following things before performing a hip replacement surgery:

  • Enquiring about the medical history of the patient.

  • Taking notes of the current medications.

  • Evaluating the knee movements and range of motion.

  • Ordering tests like blood tests, X-rays.

  • MRI is not required in most cases but in some cases, the doctor might ask to perform an MRI.

During the evaluation process, the patients can ask any queries related to medications, which medicines to terminate a week before surgery, getting clarity on the surgery process, etc.

During the Surgery
  • During the procedure, the patient is given a general anesthetic, spinal block, or nerve block to numb the nerves around the knee joint and the lower body area.

  • The surgery takes about two hours to get completed.

  • The surgeon makes an incision in the front of the knee joint, extracts the knee cap, removes the dead and damaged tissues, makes artificial joint surfaces, and replaces it with the joint implant.

After the Surgery

After the surgery, the patient might need a few hours to regain their senses.

  1. The medical staff monitor blood pressure, blood sugar level, pulse, alertness, and pain.

  2. The patient is advised to stay under the supervision of the hospital for a few days, many patients can go home on the day of surgery. The number of days spent can depend on one’s personal needs and conditions.

  3. A physical therapist might show some knee exercises to the patient.

  4. The patient is asked to move the knee and ankle area to allow blood to flow into the leg.

  5. After a few hours or a day, the patient can be asked to move and try to walk if possible to prevent the risks of blood clots.

What can be Done at Home: Pre And Post-Surgery Care And Precautions

Pre-Surgery Care
  1. The patient should stop the intake of certain medications prescribed by the doctor.

  2. Avoiding any dental treatments like teeth cleaning before the surgery.

  3. Getting all the necessary documents and things that will be required at the hospital ready.

  4. Getting proper rest and eating healthy.

  5. The patient is most likely to get nervous about the surgery process, it is better to let the family members and close ones know how they are feeling.

Post-Surgery Care:
  1. If the patient is away from home, they should arrange for someone to help them with their aftercare.

  2. Keeping the necessary items like a phone, water bottle, prescription, medicines, tissues, etc., on a bedside table or chair.

  3. Using the support of someone to go to the washrooms.

  4. Getting enough rest.

  5. Wearing light, comfortable clothes.

  6. The doctor can advise the patient to wear compression shoes to prevent swelling and pain.

  7. If unable to get a proper sleeping position, the person can use pillows to support their back and legs.

  8. While at rest, the patient should try moving the knee joint to the limited range the body allows. It will help to prevent the risks of blood clots.

  9. Receiving physical therapy after a few weeks. (note: it should be done under the supervision of any professional) or the person can do the exercises shown to them by any physical therapist or you can also contact our team to get physiotherapy at the comfort of your home.

  10. Installing handrails or safety bars in the washroom.

  11. Getting a toilet seat riser if the patient has a low toilet.

  12. Using handrails while climbing stairs.

  13. Arranging a shower bench or stool.

  14. Over-the-counter medicines like painkillers and muscle relaxants can be taken to avoid occasional pains. (note: it is advisable to contact a doctor before trying out the medications)

  15. Avoiding smoking and consumption of alcohol can help in faster recovery.

  16. Maintaining proper intake of the prescribed medicines. (note: it is advisable to contact a doctor before trying out the medications)

  17. Eating healthy and nutritious. Drinking enough water.

  18. When patients feel better and are confident about doing activities, they should start with easy and light tasks.

  19. Medical knee caps or bands can be used for extra support. (note: one should get the band after consulting their doctors).

  20. After a few weeks, the patient can do light joint exercises, go for light walks, etc.

  21. Strenuous activities should be strictly avoided as they can increase the chances of infections.

  22. Undergoing surgery is a significant take on mental health. So, patients who feel demotivated or depressed should share and let their close ones know. The family members should support them in the journey. Being patient and optimistic is the key.

  23. If faced with any signs of infection or other unexpected occurring, the patient must immediately contact their doctor.

Recovery

  • The period of recovery varies from person to person. It takes about six to twelve months to get completely healed.

  • After three to six weeks, most patients resume their daily activities, which include light tasks like walking, going out, etc.

  • If the patient feels confident in their muscular and joint strength, then they can start driving. (note: it is recommended to consult the doctor before starting)

  • Regular follow up with the doctor should be done properly and a clear idea about the medicines should be taken.

  • The patient must ensure that their knee joint is healing properly.

  • The doctor can test for any reactions and side effects due to the implant used.

  • A successful knee replacement surgery can last for 15 years and more.

  • Most patients heal well without facing any major complications. But if any signs of :

    1. Infection

    2. Blood clot

    3. Excessive pain

    4. Unexpected pressure at certain regions

    5. Change in leg length

    6. High fever and chills

    7. Loosening and

    8. If nerve damage is seen, then the patient should immediately consult their doctor or surgeon.

Hello everyone I have been regularly taking sessions from Resolve360 (from Apoorva) since January 2020. I have been suffering from upper and lower back pain from last 10 years. After being to number of doctors and physiotherapist in Delhi & Ncr, i could not find any solution to my pain. I was not even ready to try with her as i had lost all hopes and so much of money but luckily my sister pushed me alot and i started taking sessions. I found her instructions and methodology very helpful. She is updated in her academic knowledge in the field of Physiotherpy. She clarified my doubts about the role of Laser and Ultrasound treatment quoting evidence based data. With my personal experience I agree with her opinion that these are more of placebo effect.

Physiotherpy though not a miracle treatment, is a compelling necessity to most of the musculoskeletal and neurological illnesses. It needs to be accepted as healthy way to live.

Rehab for Heel Pain

From being a couch potato with poor cardio respiratory system to reaching pre-athlete levels of fitness in a span of 3 months, my journey with Resolve360 and Dr.David (Physiotherapist) has been anything but magical!

About 3 to 4 months ago, my blood report showed very high bad cholesterol levels, I was low on stamina and had frequent breathing issues. My GP prescribed me cholesterol meds for a month but I felt that alone wouldn’t get me back in shape because I needed a major lifestyle overhaul.

Dr.David educated me about my condition in detail and where it could lead me to if I don’t take action in the long run. He put me on a balanced workout regime being easy with cardio workouts in the initial days and gradually upgraded it observing me closely. He also advises me on dietary choices and follows up frequently. During the initial days, my ears used to get blocked within 5 wall squats and I stop right away but now I do over 20 in one shot and hardly feel a thing. I used to lose balance and fall off over my face within 6 push-ups or 20 seconds of plank and now I do about 24 push-ups in one shot. My heart rate used to raise above 150 bpm doing jumping jacks or other cardio workout within 20-30 seconds. You won’t believe I don’t do any of these cardio workouts without additional weights on my feet these days and my heart rate and recovery are as steady as they could get. I couldn’t run for over 2 min without stopping and gasping and now I cover about 4.6 km in 30 min and follow-up with a 20 min walk. Not convinced yet? My VO2 Max (I use an Apple Watch to measure this stat) was just about 33 while I started with Resolve360 and David. That reflects a very poor cardio respiratory fitness for a 34 year old. It’s 43.5 today ! Easily above average. Please read upon about VO2 Max and how it directly relates to cardio respiratory fitness so you get an accurate picture about my monumental progress in the last 3 months. I’ve been trying to get into a regular physical workout and balanced diet discipline since my 20s but I’d never been so motivated as I’m now. Give these guys a try. After all, the best investment you could ever make is on your health !
